摘要：目的:分析我国DRG支付方式改革对医疗安全的影响,为我国医疗体制改革、政策制定及医疗机构管理提供参考。方法:检索PubMed、Embase、Web of Science、中国知网、中国生物医学、万方数据知识服务平台、维普期刊资源整合服务平台7个数据库,检索时间为2009年1月1日-2024年12月31日。按照纳入标准与排除标准,甄别筛选所有被检索到的文献,对研究DRG和医疗安全主题的文献进行质量评价,并通过主题综合法分析纳入研究的文献。结果:共纳入13篇文献研究,汇总成1个核心主题和6项评价指标。共计1008890例研究对象,文献质量较高。系统评价结果显示,与未实施DRG组相比,实施DRG组的低风险组死亡率、中低风险组死亡率、中高风险组死亡率、住院死亡率、Ⅰ类切口手术部位感染率均呈现下降趋势,其中最大降幅达1.10%;且住院患者满意度提高,最高增幅达2.75%。结论:我国DRG支付方式改革可有效促进医疗安全,但仍存在上升空间。未来应充分发挥DRG支付方式改革导向作用,强化医疗安全指标,推动医疗安全管理标准化、精细化,推动医疗服务高质量发展。
Objective:To analyse the impact of the reform of China's DRG payment method on medical safety,and to provide reference for China's healthcare system reform,policy formulation and management of medical institutions.Methods:Seven databases were searched,PubMed,Embase,Web of Science,China Knowledge,China Biomedicine,Wanfang Data Knowledge Service Platform,and VIP Journal Resource Integration Service Platform,and the search period was from 1 January 2009 to 31 December 2024.In accordance with the inclusion and exclusion criteria,all retrieved literature was screened to evaluate the quality of literature studying the topic of DRG and medical safety,and the literature included in the study was analysed by the thematic synthesis approach.Results:A total of 13 literature studies were included,summarised into 1 core theme and 6 evaluation indicators.With a total of 1008890 study subjects,the quality of the literature was high.The results of the systematic evaluation showed that compared with the group without DRG implementation,the mortality rate of the low-risk group,the mortality rate of the medium-low-risk group,the mortality rate of the medium-high-risk group,the in-hospital mortality rate,and the rate of surgical site infections of type I incisions in the group with DRG implementation showed a decreasing trend,with the largest decrease of 1.10 per cent;and in-hospital patient satisfaction increased,with the highest increase of 2.75 per cent.Conclusion:China's DRG payment method reform can effectively promote medical safety,but there is still room for upward movement.In the future,we should give full play to the guiding role of the DRG payment method reform,strengthen medical safety indicators,promote the standardisation and refinement of medical safety management,and promote the high-quality development of medical services.
