摘要：目的系统评价温阳法结合西医常规治疗激素抵抗型肾病综合征(steroid resistant nephroticsyndrome, SRNS)的疗效及安全性。方法计算机检索CNKI、WanfangData、VIP、CBM、PubMed、EMbase、Web of science及The Cochrane Library,全面收集温阳法治疗SRNS临床性随机对照试验的中、英文文献,检索时限为建库至2020年8月,对符合标准的文献采用RevMan 5.3软件进行Meta分析。结果共纳入11项研究,总计723例SRNS患者。Meta分析显示,治疗组在提高治疗总有效率(RR=1.29,95%CI[1.19,1.39]),降低24 h尿蛋白定量(MD=-1.46,95%CI[-2.09,-0.83]),升高血浆白蛋白(MD=6.56,95%CI[3.66,9.47]),降低尿素氮(MD=-1.27,95%CI[-2.15,-0.39])、肌酐(MD=-22.84,95%CI[-39.97,-5.70])、胆固醇(MD=-1.41,95%CI[-2.10,-0.72])水平等方面明显优于对照组,且能降低1年后复发率(RR=0.34,95%CI[0.20,0.59])及不良反应发生率(RR=0.54,95%CI[0.35,0.82]),但对降低甘油三酯水平(MD=-0.29,95%CI[-0.71,0.14])与单纯西医组比较差异无统计学意义。结论已有证据显示,温阳法结合西医常规治疗SRNS在提高临床综合疗效及安全性方面显著优于单纯的西医治疗方案,值得临床应用推广。
Objective To evaluate the efficacy and safety of Wenyang Formula in the treatment of steroid resistant nephroticsyndrome(SRNS). Methods Randomized controlled trials in treatment of SRNS with Wenyang Formulawhich were retrieved by computer in the CNKI, Wanfang, VIP, CBM,PubMed, EMbase, Web of Scienceand the Cochrance Library were collected comprehensively.The searchtime was ended on August, 2020.The Meta-analysis on standards-compliantliterature was performed using RevMan5.3 software. Results A total of 11 studies were enrolledand the cases of SRNS patients totaled 723.Meta-analysis showed that the treatment groupwas better than the control group in improving the total clinical effective rate(RR=1.29,95%CI[1.19,1.39]),reducing the one year’s recurrencerate(RR=0.34,95%CI[0.20,0.59]),decreasing 24 h urine protein quantitation(MD=-1.46,95%CI[-2.09,-0.83]),elevating serum albumin(MD=6.56,95%CI[3.66,9.47]),reducing blood urea nitrogen(MD=-1.27,95%CI[-2.15,-0.39]),serum creatinine(MD=-22.84,95%CI[-39.97,-5.70])and cholesterol(MD=-1.41,95%CI[-2.10,-0.72]),and reducing the incidence of adverse reactions(RR=0.54,95%CI[0.35,0.82]),buthad no effect in reducing triglyceride.Conclusion Current evidence shows that the clinical efficacy and safety of the combination of Wenyang Formula and western medicine in treatment of SRNS is significantly better than the pure western treatment program.
