摘要：目的:通过针刺结合Bobath技术治疗缺血性中风后痉挛性偏瘫患者,观察运动功能评测表(Fugl-Meyer Assesssment,FMA)和改良Ashworth量表(Modified Ashworth Scale,MAS)治疗前后的变化,进一步判断其临床疗效。方法:将70例缺血性中风后痉挛性偏瘫患者运用随机数字分为治疗组(35例)、对照组(35例)。两组患者在年龄、性别、病程等一般资料上对比,无显著性差异(P>0.05),具有可比性。对照组采用常规针刺疗法,留针30 mins,每日1次,6天为个疗程,每个疗程隔1天,5个疗程后评价疗效。治疗组在对照组的基础上结合Bobath技术。每次训练治疗30~40 mins。每日1次,6天为1个疗程,5个疗程后评价疗效。观察治疗前后FMA、MAS量表评分差值的变化,评价治疗前后两组患者总体疗效的改变。结果:治疗后对照组和治疗组FMA、MAS量表评分均有改善(P<0.05),治疗组评分改善优于对照组,差异有统计学意义(P<0.05)。对照组总有效率为85.71%,治疗组总有效率为94.28%,治疗组疗效明显优于对照组,差异有统计学意义(P<0.05)。结论:针刺能够减缓缺血性中风后痉挛性偏瘫患者肌肉收缩,降低肌张力,Bobath技术能够增加患侧肢体的协调运动模式,针刺结合Bobath技术治疗缺血性中风后痉挛性偏瘫患者能够有效降低患侧肢体痉挛程度,提高日程生活能力,疗效显著,为临床医师治疗该病提供了新方法,值得在临床推广运用。
Objective: The patients with spasmodic hemiplegia after ischemic stroke were treated by acupuncture with Bobath technology, the changes of the motor function assessment table and modified Ashworth scale MAS were observed before and after treatment, and the clinical efficacy was further determined. Methods: 70 patients were divided into the treatment group (35 cases) and the control group (35 cases). The control group received conventional acupuncture. The treatment group was given the Bobath technology more. Results: After treatment, the FMA and MAS scores in the treatment group were better (P<0.05). The total efficiency in the control group was 85.71%, 94.28% in the treatment group (P<0.05). Conclusion: Acupuncture can reduce ischemic stroke patients with spastic hemiplegia after muscle contraction, reduces muscle tension. Bobath technology can coordinated movement. Acupuncture with Bobath technology can effectively reduce the degree of limb spasm, improve the ability life, with good efficacy, and is worth popularizing in clinic.
