摘要:体外冲击波(Extracorporeal shock wave,SW)早期在泌尿系结石的治疗中被广泛应用,其主要原理是其产生的适度当量冲击波对病变靶点异常组织的振动解离效应。慢性筋骨病是骨科筋骨系统较为常见的一种慢性疼痛性疾病,近年来诸多学者尝试将体外冲击波应用于骨科慢性筋骨病的治疗,发现其疗效显著。因此,以“筋骨并重”理论为切入点来探讨体外冲击波对慢性筋骨病的治疗效应依据充分。但其对肌肉-骨骼系统的筋骨病变的治疗机制目前尚不十分清楚。其机制可能是体外冲击波对肌肉-骨骼系统的筋骨病变靶点具有弥散式修复重建效应,将机械能量学刺激转化为生物学效应从而调整了“筋骨失衡”的生物力学紊乱及筋骨内环境失调状态,最终重建了筋骨软组织生物力学平衡。基于此,文章主要从体外冲击波对慢性筋骨病治疗的理论溯源、机制研究及临床应用等方面展开,以明确其治疗慢性筋骨病的筋骨生物力学机制,以期能够更好地指导临床。
Extracorporeal shock wave(SW)is widely used in the early treatment of urinary calculi,its main principle is the vibration dissociation effect of moderate equivalent shock wave on pathological target abnormal tissue.In recent years,many scholars have tried to apply extracorporeal shock wave(ESW)in the treatment of chronic osteoarthritis in the department of orthopedics,and found that it has a significant effect.Therefore,it is sufficient to discuss the therapeutic effect of extracorporeal shock wave on chronic skeletal and muscular diseases with the theory of“equal emphasis on muscles and bones”as the breakthrough point.However,its therapeutic mechanism on the musculoskeletal system is still unclear.The possible mechanism of extracorporeal shock wave on muscle-the bones and muscles of the skeletal system lesion targets have rehabilitation effect of dispersion type,biological effects to convert mechanical energy to learn stimulation to adjust the“physique imbalances”biomechanical disorders and environment is the imbalance in the bones and muscles,finally rebuilt the bones and muscles soft tissue biomechanical balance.Based on this,this paper mainly focuses on the theoretical traceability,mechanism research and clinical application of extracorporeal shock wave in the treatment of chronic skeletal and muscular diseases,in order to clarify the biomechanical mechanism of skeletal and muscular shock wave in the treatment of chronic skeletal and muscular diseases,in order to provide better clinical guidance.
