摘要：基于黄元御“一气周流、土枢四象”理论探析糖尿病胃轻瘫(DGP)的病机与辨治。认为DGP病机演变可概括为“中虚-土湿-木郁-金燥-水寒-络瘀”六期,治疗时当以健运斡旋中土为核心,把握木、火、金、水四象的动态变化,根据不同阶段的病机演变,立足扶阳抑阴、疏肝理气、升阳敛肺、清上温下、散瘀通络、敛神静心之法,使气机升降有序,周流不息。
This paper discusses the pathogenesis and differential treatment of diabetic gastroparesis(DGP)based on Huang Yuanyu's theory of"circulating qi and the earth pivot with four elements".It posits that the progression of DGP's pathogenesis can be summarized into six stages:central deficiency,earth dampness,wood constraint,metal dryness,water coldness,and collateral stasis.Treatment should focus on strengthening and regulating the central earth(spleen and stomach),while dynamically balancing the four elements of wood,fire,metal,and water.The treatment strategies vary across different stages and include supporting yang and suppressing yin,soothing the liver and regulating qi,uplifting yang and consolidating the lungs,clearing the upper body and warming the lower body,dispersing stasis and unblocking collaterals,and consolidating spirit and calming the mind.The ultimate purpose is to ensure orderly ascent and descent of qi movement,maintaining its continuous flow.
