摘要:目的基于贝叶斯网状Meta分析方法评价4种常用敷料对手术压疮的预防效果。方法计算机检索PubMed、Cochrane Library、Web of Science、EBSCO、中国生物医学文献数据库(CBM)、中国知网(CNKI)及万方数据库等中外文献数据资源。检索日期截止至2016年11月。纳入对比敷料预防手术压疮效果的随机对照试验(RCT)。采用Cochrane系统评价手册5.1.0版提供的偏倚风险评估工具对纳入RCT进行质量评价。数据提取和质量评价均由2名研究者独立进行。采用ADDIS、R软件进行数据分析,Stata软件进行图形绘制。结果最终纳入20个RCT,共包含2 493例患者。网状Meta分析结果显示:泡沫敷料、液体敷料对比常规护理均能有效降低压疮发生率(P<0.05);不同类型敷料间相比,薄膜敷料与泡沫敷料间的差异有统计学意义(P<0.05),其他类型敷料间的相互比较差异均无统计学意义(P>0.05)。根据ADDIS排序结果,泡沫敷料是预防手术压疮的最优方案。结论基于网状Meta分析结果和ADDIS排序结果,泡沫敷料在预防手术压疮方面的效果优于其他敷料。此外,建议未来研究应关注不同类型敷料间效果的直接比较及成本效果评价。
Objective Based on Bayesian network Meta-analysis to evaluate the effects of four different commonly used dressing on the prevention of pressure ulcers in operation patients.Methods A systematic search of PubMed、Cochrane library、Web of science、EBSCO、Chinese biomedical literature database(CBM),Chinese knowledge infrastructure(CNKI),and Wanfang Database was carried out with an end-point of November 2016.Randomized control trials(RCTs)examining the effects of dressings for prevention of pressure ulcers in operation patients were included.Methodological quality of included RCTs was assessed according to the Cochrane handbook for systematic reviews of interventions 5.1.0.All data was screened,extracted and reviewed by 2 researchers independently.ADDIS and R software were used to analyze data and stata draw graphics.Results 20 RCTs involving 2493 patients were included.The network meta-analysis showed that when compare with usual care,foam dressings and liquid dressings can effectively reduce the incidence of pressure ulcers(P〈0.05).When compared with each other,only foam dressings was superior to film dressings in reducing pressure ulcers(P〈0.05).Probability ranking according to ADDIS showed that there was a great possibility for foam dressings to prevent pressure ulcers in postoperative patient.Conclusion Based on the results of network meta-analysis and ADDIS ranking,foam dressings are superior to other dressings in the prevention of pressure ulcers in operation patients.In addition,more attention should be made regarding comparisons directly of different dressings and their cost-effective analysis.
