摘要:目的观察针刺对糖尿病前期(PD)模型大鼠骨骼肌葡萄糖转运蛋白4(GLUT4)mRNA表达及糖脂代谢的影响,探究针刺改善PD胰岛素抵抗的作用机制。方法将40只SPF级雄性Wistar大鼠采用随机数字表法分为空白组、模型组、西药组、针刺组4组,每组10只。通过高糖高脂饲料喂养联合1%链脲佐菌素溶液腹腔注射复制大鼠PD模型,模型复制成功后,针刺组选取中脘、足三里(双)穴进行针刺,干预4周后,观察各组大鼠一般情况,检测大鼠空腹血糖(FPG)、餐后2小时血糖(2hPG)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、空腹胰岛素(Fins)水平及骨骼肌组织中GLUT4 mRNA相对表达量,计算胰岛素抵抗指数(IRI)、胰岛素敏感指数(IAI),HE染色镜下观察胰腺组织病理改变。结果与空白组比较,模型组大鼠TG、TC、LDL-C含量及2hPG、Fins、IRI明显升高,IAI及骨骼肌组织中GLUT4 mRNA相对表达量明显降低,差异均有统计学意义(P<0.05或P<0.01);与模型组比较,针刺组大鼠TG、TC、LDL-C含量及2hPG、Fins、IRI明显降低,IAI及骨骼肌组织中GLUT4 mRNA相对表达量明显升高,差异均有统计学意义(P<0.05或P<0.01)。HE染色镜下观察结果显示,针刺能够减轻PD模型大鼠胰腺组织损伤程度,使胰岛细胞排列趋于整齐。结论针刺能够有效改善PD模型大鼠胰岛素抵抗,其作用机制可能与降低血糖、血脂水平,升高骨骼肌组织中GLUT4 mRNA表达,从而加快葡萄糖运转速度,缓解糖脂代谢紊乱有关。
Objective To observe the effect of acupuncture on glucose transporter 4(GLUT4)mRNA expression in skeletal muscle and glycolipid metabolism of prediabetes(PD)model rats,and explore the mechanism of acupuncture in improving insulin resistance in PD.Methods 40 SPF male Wistar rats were divided into blank group,model group,western medicine group,and acupuncture group by random number table method,with 10 rats in each group.The PD model of rats was established by feeding high sugar and high fat diet combined with intrape-ritoneal injection of 1%streptozotocin solution.After the model was successfully established,the acupuncture group was given acupuncture at Zhongwan(RN12)and Zusanli(ST36)(both sides)acupoints.After 4 weeks of intervention,the general conditions of rats in each group were observed,and the levels of fasting plasma glucose(FPG),2 hours postprandial blood glucose(2hPG),total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C),fasting insulin(Fins)and the relative expression of GLUT4 mRNA in skeletal muscle tissue were detected.The insulin resistance index(IRI)and insulin sensitivity index(IAI)were calculated.The pathological changes of pancreatic tissue were observed under microscope by HE staining.Results Compared with the blank group,the content of TG,TC,LDL-C,and 2hPG,Fins,IRI in the model group rats were significantly increased,while IAI and the relative expression of GLUT4 mRNA in skeletal muscle tissue were significantly reduced,and the differences were statistically significant(P<0.05 or P<0.01).Compared with the model group,the content of TG,TC,LDL-C,and 2hPG,Fins,IRI in the acupuncture group rats were significantly reduced,while IAI and the relative expression of GLUT4 mRNA in skeletal muscle tissue were significantly increased,and the differences were statistically significant(P<0.05 or P<0.01).The observation results under microscope by HE staining showed that acupuncture can reduce the degree of pancreatic tissue damage in PD model rats and make the arrangement of pancreatic islet cells tend to be neat.Conclusion Acupuncture can effectively improve insulin resistance in PD model rats,and its mechanism may be related to reducing blood glucose and blood lipid levels,increasing the expression of GLUT4 mRNA in skeletal muscle tissue,thereby accelerating the speed of glucose transport and alleviating the disorder of glycolipid metabolism.
