摘要：目的:观察解毒化浊方联合西药治疗2型糖尿病(T2DM)合并非酒精性脂肪肝(NAFLD)的治疗效果。方法:将T2DM合并NAFLD的患者随机分为治疗组和对照组各45例。两组均给予基础治疗,治疗组在对照组基础上给予解毒化浊方,两组均以3个月为1个疗程,观察时间为2个疗程。在治疗前后分别评价两组血糖、糖化血红蛋白(Hb A1C)、血脂、肝酶、空腹胰岛素释放水平(FINS)、胰岛素抵抗指数(HOMA-IR)和肝脏彩超、体重指数(BMI)等指标的变化。结果:治疗组总有效率明显优于对照组(P<0.01);两组治疗后FPG、2 h PG、Hb A1C均较治疗前下降(P<0.05),治疗组及对照组均能改善FINS分泌、胰岛素抵抗指数、血脂、肝功能及体重指数,且治疗组优于对照组;肝脏B超检查:治疗组治疗后脂肪肝分度明显改善(P<0.05)。结论:解毒化浊方联合西医降糖治疗T2DM合并NAFLD在改善患者的临床症状方面效果明显,同时在减轻HOMA-IR、降糖、降低体重、改善肝功能、改善NAFLD形态学等方面具有独特的优势。
Objective： To observe the clinical effect of Jieduhuazu o decoction combined with western drug for type 2 diabetes mellitus patients with nonalcoholic fatty liver disease（NAFLD）. Methods： Patients of type 2 diabetes mellitus with nonalcoholic fatty liver disease were randomly divided into experimental group（45 patients） and control group（45 patients）. Each group received basic anti-diabetes therapy, the experimental group was also treated by taking Jieduhuazuo decoction based on basic therapy. The observation time of two groups was 2 courses of treatment（3 months for a course of treatment）. Before and after treatment, blood-glucose,glycosylated hemoglobin（Hb A1C）, blood lipids, liver enzymes, fasting insulin release level（FINS）, insulin resistance index（HOMA-IR）and liver ultrasound, changes in body weight index（BMI） and other indicators were evaluated in both group. Results： The total effective rate in experimental group was better than that in control group（P〈0.01）; in both groups, FPG, 2 h PG, Hb A1 C value decreased after treatment（P〈0.05）, FINS secretion, insulin degree of resistance, blood lipids, liver function, and body mass index were improved, the experimental group were better than those in the control group; hepatic ultrasonic B showed the degree of fatty liver obviously improved（P 〈0.05） after treatment in experimental group. Conclusion： Jieduhuazuo decoction combined with western drug for type 2 diabetes mellitus patients with nonalcoholic fatty liver disease not only has obvious effects in improving the clinical symptoms, but also has unique effects in reducing insulin restriction level, blood glucose level, body weight and improving liver function and NAFLD.
