摘要：椎间盘退变(Intervertebral disc degeneration,IVDD)是诱发多种脊柱疾患共同的病理基础,亦是导致腰背部疼痛的主要因素之一。细胞外基质(Extracellular matrix,ECM)的合成及分解代谢失衡,导致胶原纤维、蛋白聚糖及其他基质的异常降解是IVDD的主要病理特征。而基质金属蛋白酶(Matrix metalloproteinase,MMPs)与基质金属蛋白酶组织抑制剂(Tissue inhibitor of matrix metalloproteinases,TIMP)在ECM的合成与分解代谢中具有重要作用。中药多成分、多途径、多靶点、整体调节等优势在治疗IVDD方面疗效确切。目前,众多国内外学者研究发现,中药有效成分及其复方制剂通过抑制MMPs的上调及促进TIMP的表达,从而达到预防及治疗IVDD的作用。通过综述MMPs与TIMP在IVDD中的作用机制及中医药通过干预MMPs与TIMP抑制ECM的异常降解的相关实验研究,以期为中医药精准防治椎间盘退变提供新思路。
Intervertebral disc degeneration(IVDD)is the common pathological basis of a variety of spinal diseases,and it is also one of the main factors leading to low back pain.The imbalance between the synthesis and catabolism of extracellular matrix(ECM),leading to the abnormal degradation of collagen fibers,proteoglycans and other matrices,is the main pathological feature of intervertebral disc degeneration.Matrix metalloproteinases(MMPs)and tissue inhibitors of matrix metalloproteinases(TIMP)play an important role in the synthesis and catabolism of ECM.The advantages of traditional Chinese medicine,such as multi-component,multi-channel,multi-target and overall regulation,are effective in the treatment of IVDD.At present,many domestic and foreign scholars have found that the active ingredients of traditional Chinese medicine and their compound preparations can prevent and treat IVDD by inhibiting the up-regulation of MMPs and promoting the expression of TIMPs.This paper reviewed the function mechanism of MMPs and TIMP in IVDD and the relevant experimental study of inhibiting ECM abnormal degradation by intervening with MMPs and TIMP,in order to provide a new idea for the precise prevention and treatment of IVDD by TCM.
