摘要：目的:运用网状Meta分析方法比较不同中药注射液治疗妊娠期高血压疾病的疗效。方法:检索PubMed、The Cochrane Library、Embase、Web of Science、万方、维普、中国知网和中国生物医学文献服务系统数据库,搜索建库至2022年2月28日发表的中药注射液治疗妊娠期高血压疾病的随机对照试验,采用RevMan 5.3、STATA 15.1软件进行数据统计分析。结果:最终纳入86项随机对照试验,共涉及8种中药注射液,包括8990例患者。网状Meta分析结果显示:①提高临床总疗效方面,曲线下累积排序概率面积(SUCRA)排名前3位的中药注射液依次为黄芪注射液、脉络宁注射液、参麦注射液;②降低收缩压方面,SUCRA排名前3位的中药注射液依次为黄芪注射液、参麦注射液、川芎嗪注射液;③降低舒张压方面,SUCRA排名前3位的中药注射液依次为黄芪注射液、川芎嗪注射液、丹参川芎嗪注射液;④降低产后出血率方面,SUCRA排名前3位的中药注射液依次为川芎嗪注射液、黄芪注射液、复方丹参注射液;⑤降低新生儿窒息率方面,SUCRA排名前3位的中药注射液依次为黄芪注射液、川芎嗪注射液、丹参注射液。结论:中药注射液联合常规治疗可提高妊娠期高血压疾病临床疗效、降低血压、减少产后出血及新生儿窒息并发症的发生。但鉴于纳入文献的质量及研究数量差异较大,故仍需开展高质量的多中心、大样本、随机双盲试验加以佐证。
Objective:Comparison of therapeutic effects of different chinese medicine injection(CMI)on hypertensive disorders complicating pregnancy(HDCP)by network Meta-analysis.Methods:PubMed,The Cochrane Library,Embase,Web of Science,WanFang Data,VIP,CNKI and CBM databases were searched to collect randomized controlled trials(RCTs)of CMI in the treatment of HDCP published from database establishment to February 28,2022.RevMan 5.3 and STATA 15.1 software were used forO statistical analysis of data.Results:The 86 RCTs involving a total of 8 CMIs including 8990 patients were included.The results of network Meta-analysis showed that:①In terms of improving the total clinical efficacy,the top 3 CMI of SUCRA were the Huangqi injection(黄芪注射液),the Mailuoning injection(脉络宁注射液)and the Shenmai injection(参麦注射液).②In terms of reducing systolic blood pressure,the top 3 CMI of SUCRA were the Huangqi injection,the Shenmai injection,and the Chuanxiongqin injection(川芎嗪注射液).③In terms of reducing diastolic blood pressure,the top 3 CMI of SUCRA were the Huangqi injection,the Chuanxiongqin injection,and the Danshen Chuanxiongqin injection(丹参川芎嗪注射液).④In terms of reducing postpartum hemorrhage rate,the top 3 CMI of SUCRA were the Chuanxiongqin injection,the Huangqi injection and the Danshen injection(丹参注射液).⑤In terms of reducing the rate of neonatal asphyxia,the top 3 CMI of SUCRA were the Huangqi injection,the Chuanxiongqin injection and the Danshen injection.Conclusion:CMI combined with conventional western medicine treatment can improve the clinical efficacy of HDCP,reduce blood pressure,reduce the occurrence of postpartum hemorrhage and neonatal asphyxia complications.However,in view of the large differences in the quality of the included literatures and the number of studies,it is still necessary to carry out high-quality multicenter,large-sample,randomized double-blind trials for supporting.
