摘要：目的:通过Meta分析系统评价中西医结合治疗膝关节骨性关节炎(Knee Osteoarthritis,KOA)的疗效。方法:以“中西医结合”“膝骨性关节炎”“退行性骨关节炎”为关键词,计算机检索中国期刊全文数据库、万方数据知识服务平台、维普中文科技期刊全文数据库、中国生物医学文献数据库,并手工检索相关文献,检索中西医结合治疗KOA的疗效。检索年限为2017年1月―2022年5月,提取文献的相关原始数据,通过Meta分析分析中西医结合及西医治疗KOA的疗效,采用敏感性分析评估结果的稳定性,用漏斗图评估文献的发表偏倚。结果:共检索中文文献441篇,根据纳入和排除标准,本次研究共纳入39篇文献,采用固定效应模型进行总效应值合并,优势比:2.309,95%置信区间为(2.038~2.616),提示中西医结合治疗KOA优于单纯西医治疗(Z=13.15,P<0.05),漏斗图显示左右基本对称,提示发表偏倚较小。结论:在治疗上,中西医结合优于单纯西医治疗;在临床治疗中医生应该结合患者病情,以“症”“证”为依据,发挥西医治疗之长、中医辨证之精,选择中西医结合治疗KOA,以此为患者提供精细化、个性化的治疗方案。
Objective:To systematicly evaluate the efficacy of the integrative medicine in the treatment of knee osteoarthritis by Meta-analysis.Methods:The integrative medicine,knee osteoarthritis and degenerative osteoarthritis were used as keywords,CNKI,Wanfang Data,VIP,Chinese Biomedical Literature Database(CBM)were searched by computer and related literature was searched manually.The curative effect of the integrative medicine on knee osteoarthritis was retrieved.The retrieval period was from January 2017 to May 2022.The relevant original data of the literature was extracted,and the efficacy of the integrative medicine in the treatment of knee osteoarthritis was analyzed by Meta-analysis.The stability of the results was evaluated by sensitivity analysis,and the publication bias of the literature was evaluated by funnel plot.Results:A total of 441 Chinese papers were retrieved.According to inclusion and exclusion criteria,a total of 39 papers were included in this study.The fixed-effect model was used to combine the total effect sizes,OR:2.309,95%confidence interval was 2.038 to 2.616,suggesting that the treatment of knee osteoarthritis in the integrative medicine was better than that in Western medicine alone(Z=13.15,P<0.05).The funnel plot showed a basic symmetry between the left and the right,suggesting a small publication bias.Conclusion:In terms of treatment,the integrative medicine is better than simple Western medicine.In clinical treatment,doctors should combine with the patient’s condition based on symptoms and syndromes,give full play to the advantages of Western medicine treatment and the essence of TCM syndrome differentiation,and choose the integrative medicine to treat knee osteoarthritis,so as to provide refined and personalized treatment for patients.
