摘要:目的:了解头霉素类抗菌药物致药品不良反应(ADR)的特点,为临床合理用药提供参考.方法:检索在中国知网、万方医学网和维普资讯公开发表的头霉素类抗菌药物致ADR的文献,收集到符合条件的文献77篇,共288例患者.对患者的性别、年龄、原发疾病、涉及药品、ADR发生时间、累及器官和(或)系统及临床表现、临床转归情况进行统计、分析.结果:288例患者中男性143例、女性140例,不详5例;41~60岁年龄段患者ADR发生率最高;主要原发疾病为呼吸系统、泌尿系统、消化系统疾病;涉及到的头霉素类抗菌药物主要有头孢西丁、头孢美唑、头孢米诺3种;其中用药时间在30min内发生ADR的患者为208例,占72.22%;ADR主要累及神经系统、循环系统、皮肤及其附件;242例患者(占84.03%)于停药及对症治疗后症状完全消失.结论:临床使用头霉素类抗菌药物时应监测ADR,以提高用药合理性,减少ADR的发生.
OBJECTIVE: To investigate the characteristics of adverse drug reactions (ADRs) induced by cephamycin antibacterial drugs so as to provide references for clinical rational use of drugs. METHODS : A total of 77 papers involving 288 patients related to ADRs induced by cephamycin antibacterial drugs meeting the inclusion criteria were collected from CNKI, Wanfang database and VIP information for statistical analysis regarding patients' gender, age, primary disease, drugs involved, time to ADR onset, organs and (or) systems involved in ADR, clinical manifestation and outcome of the ADRs etc. RESULTS : Among the total 288 patients, 143 were male and 140 female with gender unknown for another 5; patients aged 41-60 years showed the highest proportion; diseases of respiratory system, urinary system and digestive system were the main primary diseases; cefoxitin, cefmetazole and cefminox were the major cephamycins drugs; the ADRs in 208 cases (72.22%) occurred within 30 min of drug administration; the chief systems involved in ADRs included nervous system, circulatory system, skin and its appendages; ADR symptoms in 242 (84.03%) disappeared after drug withdrawal or symptomatic treatment. CONCLUSION: ADR monitoring should be performed during administration of cephamycin drugs so as to improve rationality of drug use and reduce the incidence of ADR.
