摘要：急性肺损伤(acute lung injury,ALI)是一种引起肺内皮和上皮屏障破坏的急性炎症疾病,临床表现为肺浸润、低氧血症和肺水肿,现在被重新归类为中度或轻度急性呼吸窘迫综合征(ARDS)。ALI是常见的呼吸系统疾病,更是重症患者发病和死亡的重要原因,在全球范围内病死率居高不下,其发病机制尚未完全阐明。但越来越多证据集中在炎症激活、细胞凋亡、氧化应激损伤、凝血功能异常等方面。非受体型酪氨酸蛋白激酶2(JAK2)/信号转导及转录激活蛋白3(STAT3)信号通路在调控细胞凋亡、增殖、分化和炎症反应中起重要作用。炎症细胞因子白细胞介素(IL)-6可激活JAK2/STAT3通路,介导炎症因子的进一步释放,引发级联放大的炎症反应参与ALI发生、发展等多个环节,被认为是ALI发展过程中的关键信号通路之一。本文以IL-6、JAK2/STAT3信号通路为对象,阐明其介导的信号通路在ALI发展中的作用及研究进展。
Acute lung injury(ALI)is an acute inflammatory disease that causes the destruction of the lung endothelium and epithelial barrier.The clinical manifestations are pulmonary infiltration,hypoxemia,and pulmonary edema,and ALI is now reclassified as moderate or mild acute respiratory distress syndrome(ARDS).ALI is a common respiratory disease,and is also an important cause of morbidity and death in critically ill patients.The mortality remains high worldwide,and its pathogenesis has not yet been fully elucidated,but more and more evidence focuses on inflammation activation,cell apoptosis,oxidative stress damage,abnormal blood coagulation,etc.The JAK2/STAT3 signaling pathway plays an important role in regulating cell apoptosis,proliferation,differentiation and inflammation.The inflammatory cytokine interleukin-6(IL-6)can activate the JAK2/STAT3 pathway,mediate the further release of inflammatory factors,and trigger the cascade of inflammatory response that participates in the occurrence and development of ALI,and are considered to be in the development process of ALI.Therefore,this paper focuses on the IL-6,JAK2/STAT3 signaling pathway to clarify the role and research status of the mediated signaling pathway in the development of ALI/ARDS.
