摘要：目的:系统评价少腹逐瘀汤联合西药治疗子宫内膜异位症的疗效和安全性。方法:检索Cochrane Library、PubMed、Embase、Web of Science以及中国知网(CNKI)、中国生物医学文献数据库(CBM)、重庆维普数据库(VIP)、万方数据库(WF)中已发表的相关文献。纳入少腹逐瘀汤联合西药治疗子宫内膜异位症的临床随机对照试验(RCT),按照Cochrane系统评价偏倚风险评估工具对文章的风险进行评价。利用RevMan5.3软件进行统计分析。结果:共纳入11篇RCT文献,共961例患者,Meta分析显示:①总有效率,试验组优于对照组[RR=1.17,95%CI(1.11,1.23),P<0.00001];②血清糖类抗原125(CA125)水平,试验组治疗后低于对照组[MD=-11.17,95%CI(-20.78,-1.56),P=0.02];③疼痛视觉模拟评分法(VAS)评分,治疗后试验组评分低于对照组[SMD=-1.59,95%CI(-2.17,-1.01),P<0.00001];④复发率,试验组低于对照组[RR=0.32,95%CI(0.19,0.55),P<0.0001];⑤不良反应发生率,试验组低于对照组[RR=0.53,95%CI(0.29,0.97),P=0.04]。结论:少腹逐瘀汤联合西药治疗子宫内膜异位症与单纯运用西药对比具有更好疗效,治疗后血清CA125水平、疼痛VAS评分更低,且能够降低复发率及不良反应发生率。
Objective:To systematically evaluate the efficacy and safety of Shaofu Zhuyu Decoction(少腹逐瘀汤)combined with western medicine in the treatment of endometriosis.Methods:Cochrane Library,PubMed,Embase,Web of Science,CNKI,CBM,VIP databases,Wanfang databases online were searched for published related literature.Clinical randomized controlled trials(RCTs)were included in the treatment of endometriosis with Shaofu Zhuyu Decoction combined with western medicine.The quality of the studies were evaluated according to the Cochrane systematic evaluation bias risk assessment tool.Statistical analysis was performed using RevMan 5.3 software.Results:A total of 11 RCTs were included,with a total of 961 patients.The results of meta-analysis showed:①Total effective rate:the total effective rate of the experimental group was better than that of the control group[RR=1.17,95%CI(1.11,1.23)],P<0.00001].②CA-125 level after treatment:The level of CA-125 in the experimental group was lower than that in the control group[MD=-11.17,95%CI(-20.78,-1.56),P=0.02].③VAS scores after treatment:The VAS scores of the experimental group was lower than that of the control group[SMD=-1.59,95%CI(-2.17,-1.01),P<0.00001].④Recurrence rate:The recurrence rate of the experimental group after treatment was lower than that of the control group[RR=0.32,95%CI(0.19,0.55),P<0.0001].⑤Adverse reactions:The incidence of adverse reactions in the experimental group was lower than that in the control group[RR=0.53,95%CI(0.29,0.97),P=0.04].Conclusions:Shaofu Zhuyu Decoction combined with western medicine has better curative effect than using western medicine alone in the treatment of endometriosis.The CA-125 level and VAS scores are lower after treatment,and the recurrence rate and adverse reaction rate can be reduced.
