摘要:目的研究从肺癌脑转移瘤的MRI表现预测肺癌原发灶的组织学病理类型。方法回顾性分析经病理或临床证实的141例不同病理类型的肺癌脑转移瘤的MRI表现,分析病灶周围水肿范围、病灶大小、数量、信号特点、部位、强化方式。结果141例病例中,共347个病灶,好发于皮层下转移瘤以肺腺癌多见,结节状强化的以肺腺癌、肺小细胞癌多见,病灶环形强化的以神经内分泌癌及黏液性肺腺癌多见,斑片状强化的以肺腺癌、EGFR19基因突变型多见,囊性转移瘤以黏液性肺腺癌、神经内分泌癌多见,病灶较大的转移瘤以神经内分泌癌多见,出血多见肺腺癌。扩散加权成像(DWI)呈低信号多以神经内分泌癌多见。结论肺癌脑转移瘤的MRI表现差异与肺癌的病理类型有关系,从肺癌脑转移瘤MRI表现可以一定程度预测原发灶肺癌的组织病理学类型。
Objective To study the prediction of histopathological types of primary lung cancer by MRI findings of brain metastases.Methods MRI findings of 141 cases of different pathological lung cancer with brain metastases confirmed by pathology or clinic were analyzed retrospectively,and the extent of per-lesional edema,size,number,signal characteristics,location,and enhancement pattern of the lesions were analyzed.Results Of the 141 cases,347 lesions were found in total.Lung adenocarcinoma showed the most common subcortical metastasis,lung adenocarcinoma and small cell carcinoma showed the most common nodular enhancement,neuroendocrine carcinoma and mucinous lung adenocarcinoma showed the most common ring enhancement,lung adenocarcinoma and EGFR19 gene mutation showed the most common patchy enhancement,mucinous lung adenocarcinoma and neuroendocrine carcinoma showed the most common cystic metastasis,neuroendocrine carcinoma showed the most common metastasis with larger lesion,and lung adenocarcinoma showed the most common hemorrhage.Diffusion weighted imaging(DwI)showed low signal intensity mostly in neuroendocrine carcinoma.ConclusionThe difference of MRI findings of brain metastases from lung cancer is related to the pathological type of lung cancer.The MRI findings of brain metastases from lung cancer can predict the histopathological type of primary lung cancer to some extent.
