摘要:目的:观察脱管散对大鼠压力性损伤创面愈合及创面组织TNF-α、IL-6表达的影响。方法:将180只SPF级Wistar大鼠随机分出30只大鼠为空白对照组,其余150只大鼠建造I/R损伤大鼠压力性损伤模型,判断造模成功后随机分为模型组、湿润烧伤膏组及脱管散低、中、高剂量组,每组各30只。分别于连续用药后第3天、7天、14天计算创面愈合率,采用免疫荧光技术检测创面组织中TNF-α和IL-6的表达水平。结果:湿润烧伤膏组和脱管散各组的创面愈合率均高于模型组(P<0.05),除用药后第3天,脱管散中剂量组无显著优势(P>0.05)外,在用药后第3天、7天、14天,脱管散低剂量组与湿润烧伤膏组均可有效促进创面愈合(P>0.05);脱管散低剂量组优于中、高剂量组(P<0.05)。创面组织TNF-α、IL-6表达:除空白组外,各组TNF-α、IL-6表达在用药后第3天、7天、14天均呈先升高、后下降的“单峰”趋势,高峰出现在用药后第7天。模型组3个时间节点均高于其他各组(P<0.05);湿润烧伤膏组3个时间节点均低于脱管散各组(P<0.05),与脱管散低剂量组效果相当(P>0.05);脱管散各组3个时间节点低剂量组均优于中、高剂量组(P<0.05)。结论:在不同时间点,低剂量脱管散与湿润烧伤膏均具有促进创面愈合,缩小创面面积,提高创面愈合率的优势;伴随创面愈合进展,脱管散对TNF-α与IL-6的表达具有双向调节作用,即创面修复早期上调TNF-α与IL-6表达以启动免疫反应、吞噬微生物及坏死组织为创面愈合营造良好微环境,创面愈合后期下调TNF-α与IL-6表达以促进创面由炎症期向增殖期过渡,对促进压力性损伤创面愈合具有一定优势。
Objective:To observe the effect of Tuoguan San on wound healing and the expression of TNF-α and IL-6 in wound tissue of rats with pressure injury.Methods:80 SPF Wistar rats were randomly divided into 30 rats as the blank control group,and the other 150 rats were constructed into the rat pressure injury model of I/R injury.After modeling,the rats were randomly divided into model group,MEBO group,and Tuoguan San low-dose,medium-dose and high-dose groups,with 30 rats in each group.The wound healing rate was calculated on the 3d,7d,and 14d after continuous medication,and the expression levels of inflammatory factors TNF-α and IL-6 in wound tissues were detected by immunofluorescence(IF).Results:Wound healing rate:the MEBO group and Tuoguan San groups were higher than that of the model group(P
