摘要:目的探讨固本增骨方联合高压氧疗对骨质疏松症骨代谢平衡的影响及可能机制。方法50只雌性Wistar大鼠随机分为空白组、模型组、高压氧组、固本增骨方组、联合组,每组10只。除空白组外,其余各组大鼠采用去卵巢法建立骨质疏松症大鼠模型。造模成功后空白组、模型组、高压氧组大鼠给予30ml/kg生理盐水灌胃,固本增骨方组及联合组给予固本增骨方混悬液4.6g/(kg·d)灌胃,高压氧组、联合组大鼠每天灌胃后进行高压氧治疗。干预12周后检测大鼠血清骨代谢标志物[包括骨特异性碱性磷酸酶(BALP)、核结合因子α1(CBF-α1)、Ⅰ型胶原羧基末端肽(CTX-Ⅰ)、Ⅰ型前胶原氨基端前肽(PⅠNP)、骨钙素(OC)]含量变化;显微CT影像观察大鼠股骨骨密度水平及骨小梁微结构;检测大鼠下丘脑和股骨组织中骨保护素(OPG)、核因子κB受体活化因子配体(RANKL)蛋白和mRNA表达。结果与空白组比较,模型组大鼠骨密度及BALP、CBF-α1、CTX-Ⅰ、PⅠNP、OC含量、相对骨体积分数、骨小梁数量、骨小梁厚度、连接密度降低,骨小梁分离度、结构模型指数升高,下丘脑及股骨组织中OPG蛋白和mRNA表达下降、RANKL蛋白和mRNA表达升高(P<0.05)。与模型组比较,高压氧组、固本增骨方组和联合组大鼠各指标均显著改善(P<0.05);且固本增骨方组和联合组各指标改善优于高压氧组,联合组各指标改善优于固本增骨方组(P<0.01)。结论固本增骨方联合高压氧疗能够调节骨质疏松症模型大鼠骨代谢平衡,增加骨量,改善骨小梁微结构,其机制可能与调控OPG/RANKL信号通路有关。
Objective To explore the effect of Guben Zenggu Formula(固本增骨方)combined with hyperbaric oxygen therapy on imbalances in bone metabolism of osteoporosis,as well as the possible mechanism underling the effect.Methods Fifty female Wistar rats were randomly divided into a blank group,a model group,a hyperbaric oxygen group,an herbal formula group,and a combination group,with 10 rats in each group.Ovariectomy method was used to establish the osteoporosis model in all groups except for blank group.After successful modeling,rats in the blank group,model group and hyperbaric oxygen group were all given 30 ml/kg normal saline by gavage;herbal suspension of Guben Zenggu Formula at a dose of 4.6 g/(kg·d)by gavage and hyperbaric oxygen were administered in herbal formula group and hyperbaric oxygen group,respectively;the combination group received both herbal suspension and hyperbaric oxygen.After 12 weeks of intervention,the bone metabolism markers were detected in serum including bone-specific alkaline phosphatase(BALP),nuclear binding factor-α1(CBF-α1),type I collagen carboxy-terminal peptide(CTX-Ⅰ),typeⅠprocollagen amino-terminal propertied(PⅠNP)and osteocalcin(OC);micro CT imaging was used to detect femur bone density and bone trabecular microstructure;The protein and mRNA expression of osteoprotegerin(OPG)in rat hypothalamus and femoral tissue,and receptor activator of nuclear factor-kappa B ligand(RANKL)in rats hypothalamus and femur tissues.Results Compared to blank group,the model group had lower levels of bone density,BALP,CBF-α1,CTX-I,PINP,OC,relative bone volume fraction,trabecular bone number,thickness,and connection density,as well as higher trabecular bone separation,and structural model index;the OPG protein and mRNA expression significantly decreased while the RANKL protein and mRNA expression increased(P<0.05).All the outcomes showed significant improvements in hyperbaric oxygen group,herbal formula group,and combination group compared to those in model group(P<0.05);the herbal formula group and combination group showed better improvements compared to hyperbaric oxygen group,and the combination group showed the biggest changes(P<0.01).Conclusion Guben Zenggu Formula combined with hyperbaric oxygen therapy can regulate the imbalances in bone metabolism,increase bone mass,and improve the trabecular bone microstructure,possibly through regulating the OPG/RANKL signaling pathway.
