摘要：目的观察不同肺复张(RM)方法对多源性ARDS家兔的治疗作用。方法2014年1—6月选择健康成年雄性家兔48只,随机分为3组,3组采用不同的方法造模,每组造模16只,造模成功后,将各组随机分为4个亚组每组4只兔子。每个亚组分别给予控制性肺膨胀法(SI),呼吸末正压递增法(IP),压力控制法(PCV)及容量控制通气模式(VCV,对照组)。分别于RM前及RM后1min、30min、1h、2h、4h监测PaO_2/FiO_2,动态肺顺应性(Cdyn),中心静脉压。结果3种RM方法对于ARDS家兔Cdyn和中心静脉压影响与对照组及3种RM方法之间比较差异无统计学意义(P>0.05);3种RM方法对于ARDS家兔PaO_2/FiO_2影响均优于对照组(P<0.05),IP法ARDS家兔在RM后1min(T_1)及RM后30min(T_2)PaO_2/FiO_2水平均显著高于同期SI组、PCV组及对照组;差异有统计学意义(P<0.05);30min后3种RM方法对家兔PaO_2/FiO_2水平的改善作用比较组间差异无统计学意义(P>0.05)。结论3种RM方法对于多源性ARDS家兔均有治疗作用,其中呼吸末正压递增法在RM后早期效果优于其他两种方法。
Objective To observe the effct of different lung recruitment maneuver methods in treatment of multifocal ARDS rabbits.Methods 48 cases of healthy adult male rabbits from January to June 2014 were randomly divided into three groups,and the three groups used different methods for molding,with 16 cases in each,after the succesful molding,each group was randomly divided into four subgroups with 4 cases in each,each subgroup was respectively given the SI,IP,PCV and VCV(the control group),and the PaO2/FiO2,Cdyn and central venous pressure were monitored before RM and at 1 min,30 min,1 h,2 h,4 h after RM.Results The differences in the effects of the three RM methods on the Cdyn and central venous pressure between the thee groups and the control group and between the three groups were not statistically significant(P>0.05);the effect of three RM methods on the PaO2/FiO2 of ARDS rabbits was better than that in the control group(P<0.05);the PaO2/FiO2 levels of IP method ARDS rabbits at 1 min(T1)and 30 min(T2)were obviously higher than those in the SI group,PCV group and control group at the same period,the differences were statistically significant(P<0.05);there were no significant differences in the improvement effects of three RM methods on the PaO2/FiO2 levels of rabbits(P>0.05).Conclusion The three RM methods have a treatment effect on the multifocal ARDS rabbits,and the early effect of increase progressively peep after RM is better than that of the other two methods.
