摘要：目的:观察脱管散联合生肌散促进四肢多重耐药菌感染性创口愈合的临床疗效。方法:将符合纳入标准的85例四肢多重耐药菌感染性创口的患者,按随机数表法随机分为对照组(42例)、治疗组(43例),给予相应治疗后,治疗4周时观察两组患者临床疗效,治疗2、4周时计算创口面积、伤口床评估系统(WBS)评分、外周血Toll样受体4(TLR4)、肿瘤坏死亡因子-α(TNF-α)、白细胞介素(IL)-1β、IL-6水平检测,治疗8周时统计创口愈合的总病例数,并统计平均愈合时间。结果:对照组最终收集到39例资料,治疗组最终收集到38例资料。经治4周时治疗组总有效率(71.1%,27/38)显著高于对照组(53.8%,21/39)(P<0.05),治疗2、4周时两组创口面积较治疗前减小、WBS评分升高、外周血TLR4、TNF-α、IL-1β、IL-6水平显著降低(P<0.05),治疗4周时两组患者上述指标较治疗2周时显著改善(P<0.05),且治疗组较对照组显著改善(P<0.05);治疗8周时治疗组总愈合例数高于对照组(P<0.05),观察组平均愈合时间短于对照组(P<0.05)。结论:脱管散和生肌散联合应用治疗四肢多重耐药菌感染性创口,能有效抑制耐药菌的生长繁殖,减轻创口炎症反应,进而有效促进了伤口愈合,提高临床疗效。
Objective:To observe the clinical efficacy of Tuoguan Powder combined with Shengji Powder in promoting wound healing of limb multi drug resistant bacteria infection.Methods:Multi drug-resistant bacterial infections in limbs of 85 patients were randomly divided into control group(42 cases)and observation group(43 cases)according to the random number table method.After corresponding treatment,the clinical efficacy of the two groups was observed at 4 weeks of treatment.Wound area,wound bed assessment system(WBS)score,peripheral blood TLR4,TNF-α,IL-1βand IL-6 were calculated at 2 and 4 weeks of treatment.After 8 weeks of treatment,the total number of cases of wound healing and the average healing time were counted.Results:The data of 39 patients in the control group and 38 patients in the observation group were collected.After 4 weeks of treatment,the total effective rate of the observation group was 71.1%(27/38),which was higher than that of the control group(53.8%,21/39)(P<0.05).At 2 weeks and 4 weeks of treatment,the wound area of the two groups decreased,the WBS score increased,and the levels of TLR4,TNF-α,IL-1βand IL-6 in the peripheral blood decreased(P<0.05),and the above indexes in the two groups were improved after 4 weeks of treatment(P<0.05),and the observation group was significantly improved than the control group(P<0.05);the treatment lasted for 8 weeks,the total number of healing cases in the observation group was higher than that in the control group(P<0.05),and the average healing time in the observation group was shorter than that in the control group(P<0.05).Conclusion:Tuoguan Powder combined with Shengji Powder application in the treatment of limbs multi drug resistant bacteria infection wound can effectively inhibit the growth and reproduction of drug-resistant bacteria,reduce the wound inflammatory reaction,and effectively promote the wound healing,improve the clinical efficacy.
