摘要：中医手法是治疗腰椎间盘突出症(LDH)保守疗法中的优势疗法,具有操作简单、创伤小、疗效显著、安全性高等优势,临床应用性好。但手法操作种类繁多、命名不统一、不同的学者对其适应症及禁忌症的把握不一致,影响了手法的标准化及推广。本文搜集整理了近年来手法治疗LDH的相关文献,从LDH的诊断标准,手法的分类和命名、体位、操作步骤,治疗周期、联合疗法、疗效评价、安全性等方面进行了综述,总结目前临床上治疗LDH的常见手法的特点以及存在的一些问题,并提出展望,以期为手法的标准化和规范化建设和推广提供思路和借鉴。
TCM manipulation is the dominant conservative therapy for lumbar disc herniation(LDH).It has the advantages in simple operation,less trauma,significant curative effect,high safety and good clinical application.However,the wide variety of techniques,the inconsistent naming and understanding of their indications and contraindications by different scholars have affected the standardization and promotion of the manipulations.This study collected and sorted out relevant literature on manual treatment of LDH in recent years,and reviewed the diagnostic criteria of LDH,the classification and naming of the manipulations,positioning,operation procedure,treatment duration,combination therapy,efficacy evaluation and safety.It summarized the characteristics of common methods for the treatment of LDH and some existing problems in clinical practice,and proposed prospects with a view to providing ideas and references for the standardization and standardized construction and promotion of the methods.
