摘要：目的探讨永久性肠造口患者照顾者护理过程中的真实感受和情感体验。方法采用目的抽样选取永久性肠造口患者照顾者4例为研究对象,采用无结构式的访谈法对个案进行深入访谈,运用三维度叙事分析法对照顾者护理过程进行叙事分析。结果照顾者对护理过程的感受包括围手术期表现出对疾病的不确定感;独自护理患者初期表现出较强的无助感;照顾者变得自信,认为患者护理过程是自身的成长过程。结论应根据患者病情和照顾者自身需求,准确评估照顾者知识掌握度和照护能力,有针对性地为照顾者开展疾病及肠造口健康知识教育,为照顾者提供更好地提供指导和支持,减轻照顾者身心负担。
Objective To explore the caregivers’experience of nursing process for patients with permanent colostomy.Methods Four caregivers of patients with permanent colostomy were selected by purposive sampling.Qualitative research was conducted using unstructured in-depth interviews.Data was analyzed using the three-dimension space approach model of Clandinin and Connelly.Results Caregivers had reported a feeling of uncertainty about the disease during the perioperative period;Caregivers suffered from a sense of helplessness in the early stage of care;caregivers became more confidence and considered nursing as a process of self-growth.Conclusion Targeted health education about stoma-related knowledge should be carried out for caregivers.It is required to assess the disease condition of patients and caregivers’health needs,ostomy knowledge and care ability before taking out targeted health education.More efforts should be made to support the caregivers and reduce their physical and psychological distress.
