摘要:目的通过复合造模方法,初步探索构建一种湿热毒盛型糖尿病足大鼠模型。方法在高脂饲料结合20%蔗糖水自由饮用、高温高湿的饲养条件下,经链脲佐菌素(Streptozotocin,STZ)诱导建立糖尿病大鼠模型的基础上,在足底手术致创并注射金葡萄球菌造成糖尿病足溃疡大鼠模型,造模成功后,采用爱可欣和生肌玉红膏纱条干预治疗来验证模型是否成功。观察造模前后大鼠的一般情况、体重、血糖改变、足底创口的改变,初步建立湿热毒盛证候评分表评价,取足底溃疡创面和肌腱进行苏木素-伊红(HE)染色;酶联免疫吸附测定法(ELISA)检测各组大鼠溃疡创面、肌腱组织和血清中的白细胞介素-6(Interleukin-6,IL-6)、肿瘤坏死因子-α(Tumor necrosis factor-α,TNF-α)、C反应蛋白(C-reactive protein,CRP)等炎症因子的含量。结果4组大鼠在一般情况、体重、血糖、足底创口改变、HE染色、炎症因子水平等方面有显著差异。与假手术组、爱可欣组、生肌玉红组相比,模型组一般状态差,体重下降,血糖升高,湿热毒盛证候评分明显升高,差异具有统计学意义(P<0.01);HE染色结果表明:模型组溃疡创面和肌腱组织有更多炎症细胞浸润,胶原纤维排列混乱更甚;溃疡创面、肌腱组织和血清中的IL-6、TNF-α、CRP等炎症因子的含量明显升高(均P<0.01)。经爱可欣及生肌玉红膏纱条干预治疗后,爱可欣组血清和溃疡创面、肌腱组织中的IL-6、TNF-α、CRP等炎症因子的含量较生肌玉红组降低(P<0.01);生肌玉红组较爱可欣组创面愈合率明显升高(P<0.01)。结论采用多因素造模方法初步构建了湿热毒盛型糖尿病足溃疡动物模型。
Objective To explore and construct a rat model of diabetic foot with damp-heat toxin by compound modeling method.Methods Under the condition of free drinking of high fat diet combined with 20%sucrose water,high temperature and high humidity,the diabetic rat model was induced by streptozotocin(Streptozotocin,STZ).After the rat model of diabetic foot ulcer was induced by plantar operation and injection of Staphylococcus aureus,Aikexin and Shengji Yuhong ointment gauze were used to verify the success of the model.The general condition,body weight,blood glucose and plantar wound of rats before and after modeling were observed,and the score table of damp-heat toxin syndrome was established.The wound and tendon of plantar ulcer were stained with hematoxylin-eosin(HE).The contents of inflammatory factors such as interleukin-6(Interleukin-6,IL-6),tumor necrosis factor-α(TNF-α)and Creactive protein(C-reactive protein,CRP)in ulcer wound,tendon tissue and serum of rats in each group were detected by enzyme-linked immunosorbent assay(ELISA).Results There were significant differences in general condition,body weight,blood glucose,changes of plantar wound,HE staining and the level of inflammatory factors among the four groups.Compared with sham operation group,Aikexin group and Shengji Yuhong group,the general state,weight loss,blood glucose and damp-heat toxin syndrome score of model group were significantly higher than those of sham operation group,Aikexin group and Shengji Yuhong group,The difference was statistically significant(P<0.01).The results of HE staining showed that there were more inflammatory cells infiltration and collagen fibers in ulcer wound and tendon tissue of model group.The contents of inflammatory factors such as IL-6,TNF-αand CRP in ulcer wound,tendon tissue and serum were significantly increased(P<0.01).After intervention with Aikexin and Shengji Yuhong ointment,the contents of inflammatory factors such as IL-6,TNF-αand CRP in serum,ulcer wound and tendon tissue in Aikexin group were lower than those in Shengji Yuhong group(P<0.01),and the wound healing rate in Shengji Yuhong group was significantly higher than that in Aikexin group(P<0.01).Conclusion The animal model of diabetic foot ulcer with damp-heat toxin was established by multi-factor modeling method.
