摘要:目的:基于白睛成像人工智能技术分析高血压性视网膜病变(HRP)白睛眼象的相关性。方法:纳入2023年5-6月甘肃中医药大学附属医院住院部高血压(HBP)患者100例,其中HRP组58例,HBP组42例。两组使用“MyEyeD-l0白睛无影成像健康智能分析系统”对右眼进行白睛眼象特征的高清采集、特征提取和综合分析,采集目络特征形态、血脉颜色、病变分布区域,同时观察并分析两组的血清脂联素(APN)、眼压、视力、视网膜动静脉比值(AVR)、视网膜黄斑中心凹厚度(MCT)与目络特征的相关性。结果:HRP组主要目络形态斑、血脉特征分值,血脉颜色黯红色特征分值,目络区域特征A区、H区均显著高于HBP组(P<0.05,P<0.01)。HRP组血清APN与视网膜AVR显著低于HBP组,且与高血压视网膜病变程度分期呈负相关;HRP组眼压显著高于HBP组,且与高血压视网膜病变程度分期呈正相关。HRP组斑与血清APN呈负相关;A区特征分值与视网膜AVR呈负相关;血脉、H区特征分值与眼压呈正相关;黯红色、H区特征分值与视网膜AVR呈负相关。结论:HRP引起目络疾病的机制是可能与机体伤于脾胃和肝,脾失运化,肝失疏泄,久则痰湿浊瘀内生,气滞痰瘀内停浮现于目络而出现“斑”“血脉”“黯红色”形成有关联,其目络特征的形成考虑与血清VPN的降低、眼压的升高及视网膜AVR降低的微血管病变机制有关。
Objective:To analyze the correlation of white eye in hypertensive retinopathy(HRP)based on visual differentiation and artificial intelligence.Methods:A total of 100 patients with hypertension(HBP)in the Affiliated Hospital of Gansu University of Chinese Medicine from May to June 2023 were enrolled,including 58 in the HRP group and 42 in the HBP group.The two groups used the ‘MyEyeD-10 White eye shadowless imaging health intelligent analysis system'to conduct highdefinition acquisition,feature extraction and comprehensive analysis of the white eye image features of the right eye,collect the features of eye collages,blood vessel color,and lesion distribution area.The correlation between serum adiponectin(APN),intraocular pressure,visual acuity,retinal arteriovenous ratio(AVR),macular fovea thickness(MCT)and ocular features of the two groups was also observed and analyzed.Results:The spot and blood characteristic scores of main visual and collared spots,blood color dark red,eye region A and H in HRP group were significantly higher than those in HBP group(P<0.05,P<0.01).Serum APN and retinal AVR in HRP group were significantly lower than those in HBP group,and were negatively correlated with the degree and stage of hypertensive retinopathy.The intraocular pressure in HRP group was significantly higher than that in HBP group,and was positively correlated with the degree and stage of HRP.There was a negative correlation between plaque and serum APN in HRP group.The feature scores of region A were negatively correlated with retinal AVR.Blood vessel and H region characteristic scores were positively correlated with IOP.The characteristic scores of dark red and H region were negatively correlated with retinal AVR.Conclusion:The mechanism of eye and collagal diseases caused by HRP may be related to the injury of the spleen,stomach and liver,the loss of spleen transport,the loss of liver drainage,the endogenous phlegm turbidity and stasis,and the formation of‘spots',‘blood vessels'and‘dark red'caused by qi stagnation and phlegm stasis.The formation of the ocular features is considered to be related to the mechanism of microangiopathy caused by the decrease of serum VPN,the increase of IOP and the decrease of retinal AVR.
