摘要：目的分析《针灸大成》中肺系疾病的诊疗规律及古今相较异同点。方法采用Microsoft Office Excel 2019摘录《针灸大成》肺系疾病中感冒、咳嗽、哮喘、肺痈、肺痨的相关条目,对高频选穴、所属经脉、特定穴及诊治方法建立数据库,进行频次分析。结果《针灸大成》中有关肺系疾病的相关条目共182条,其中涉及穴位124个,使用总频次314次;在治疗肺系疾病中,足太阳膀胱经、足阳明胃经取穴数最多,频率均为12.90%,经脉使用频次居前三的依次为足太阳膀胱经、足阳明胃经、手太阴肺经,频率分别为17.20%,14.65%,14.65%;四肢部位穴位的使用个数(55个)及频次(161次)最高,特定穴应用最多的是五输穴(使用个数34个,频次110次)。结论在诊治思路、取穴规律方面,《针灸大成》诊治肺系疾病与现代医学具有共性,但在针、灸的量效关系及具体应用中存在差异,亟待后续进行针对性研究。
Objective To analyze the diagnostic and therapeutic principles of pulmonary diseases in Zhenjiu Dacheng and the similarities and differences between ancient and modern approaches.Methods The related entries of pulmonary diseases such as common cold,cough,asthma,pulmonary abscess,and pulmonary tuberculosis in Zhenjiu Dacheng were extracted using Microsoft Office Excel 2019,and a database was established for high-frequency acupoints,corresponding meridians,specific acupoint attributes,and therapeutic methods.Frequency analysis was conducted.Results There were a total of 182 related entries of pulmonary system diseases in Zhenjiu Dacheng,involving 124 acupoints with a total frequency of 314 times.In the treatment of pulmonary diseases,the most frequently used acupoints were the ones on the foot taiyang bladder meridian and the foot yangming stomach meridian,both of which accounted for 12.90%.The frequency of the three meridians chosen in order were the foot taiyang bladder meridian,the foot yangming stomach meridian,and the hand taiyin lung meridian,accounting for 17.20%,14.65%,and 14.65%,respectively.The highest number of acupoints used(55)and the highest frequency of usage(161)were on the limbs.The most frequently used specific acupoints were the five-element acupoints,with a total of 34 and 110 usage times,respectively.Conclusion In terms of diagnosis,treatment and acupoint selection rules,the treatment of pulmonary diseases in Zhenjiu Dacheng shares similarities with modern medicine,but there are diffe-rences in the relationship between acupuncture and moxibustion and their specific applications,which require further targeted research.
