摘要：目的:探究区块链在全球卫生健康领域的发展现状、热点与前沿。方法:采用文献计量学的方法,以Embase和Mediline数据库为数据源,系统检索与分析从建库至2020年8月17日收录的区块链在全球卫生健康领域应用的相关文献。使用Excel 2019统计分析该领域相关文献的年发文量、发表国家、机构、作者和来源期刊等基本特征,使用VOSviewer 1.6.9软件统计分析并构建国家间/机构间/作者间合作关系可视图,进行关键词共现网状聚类分析并绘制关键词热点图。结果:经两位研究人员独立筛选文献并交叉核对后,最终纳入区块链在卫生健康领域应用相关文献157篇,包含39个国家的279个研究机构的523位作者,第1篇纳入文献出现于2016年。其中发文量居前3的国家分别是美国、中国和印度,发文量最多的作者是来自加州大学圣地亚哥分校的Kuo TT。157篇文献共刊载于69种期刊上,刊文量前3的期刊分别是Journal of Medical Internet Research(n=24),Journal of Medical Systems(n=17),Sensors(n=13)和Studies in Health Technology and Informatics(n=13)。关键词聚类分析发现区块链在卫生健康领域的5个亚研究方向,分别是个人健康记录系统、远程移动医疗、临床试验数据共享、药品供应链追溯、基因数据库建设。电子健康记录、医疗信息系统、医疗记录、算法等是目前的研究热点。结论:欧美等发达国家对区块链在卫生健康领域中的应用研究处于领先地位,同时中国、印度等发展中国家也很重视这一领域的发展。现阶段区块链在卫生健康领域中的应用多处于理论或框架协议的建设探索,研究热点多聚焦在电子健康记录、个人医疗记录和临床试验数据处理等方向。更多机构在这一领域的系列研究和机构间合作研究将在未来切实改变现有卫生健康医疗保健数据的储存获取共享方式。
Objective:To explore the development status,hot spots and frontier of blockchain in healthcare field.Methods:Using Embase and Mediline databases as data sources,related literatures of blockchain in healthcare field were systematically searched and analyzed.The literature search date was August 17,2020.Excel 2019 was used to analyze the basic characteristics,such as the number of published papers,published countries,institutions,authors and source journals.VOSviewer 1.6.9 was used to analyze and construct the visual map of the cooperation relationship between countries/institutions/authors.The keyword co-occurrence network clustering analysis was carried out and the keywords hot spot map was drawn.Results:After two researchers independently screened and cross checked the literatures,157 literatures related to the application of blockchain in healthcare field were finally included,including 523 authors from 279 institutions in 39 countries.The first included literature appeared in 2016.The top three countries in the number of published papers are the United States,China and India,and Kuo TT from the University of California,San Diego is the most prolific author.157 papers were published in 69 journals.The top three journals of paper numbers were Journal of Medical Internet Research(n=24),Journal of Medical Systems(n=17),Sensors(n=13)and Studies in Health Technology and Informatics(n=13).Key words cluster analysis found five sub-research directions of blockchain in healthcare field,including personal health record system,remote mobile medicine,clinical trial data sharing,drug supply chain traceability and gene database construction.Electronic health records,medical information systems,medical records,algorithms are the current research hotspots.Conclusion:Developed countries such as Europe and the United States take the lead in the research on the application of blockchain in the field of healthcare,while developing countries such as China and India also attach great importance to the development in this field.At present,the application of blockchain in the field of healthcare is mostly in the exploration of the construction of theory or framework protocol,and the research focuses on electronic health records,personal medical records and clinical trial data processing.The series research and inter agency cooperation research of more institutions in this field will change the existing way of storage,access and sharing of health care data in the future.
