摘要：目的比较3种手术方法治疗胫骨干GustiloⅡ型骨折的疗效。方法 75例胫骨干GustiloⅡ型骨折患者按治疗方法分为3组:A组(25例)采用交锁髓内钉内固定;B组(25例)采用锁定钢板螺钉内固定;C组(25例)采用外固定支架固定。术后定期随访,膝踝关节功能恢复采用Johner-Wruhs评分标准进行功能评定。结果 75例均获得随访,时间10～36(25±1.5)个月,A、B两组在膝踝关节功能恢复、部分负重时间、胫骨骨折愈合时间等方面优于C组(P<0.05)。A、B、C 3组优良率分别为94.6%、94.0%、81.4%。结论交锁髓内钉和锁定钢板治疗胫骨干GustiloⅡ型骨折疗效较好。
Objective To compare the clinical results of three devices operative procedures for the tibial fractures of Gustilo grade-Ⅱ. Methods 75 case were enrolled in this study：25 patients,group A,were treated by interlocking intramedullary nail ;25 patients, group B, were treated by locking compression plate （LCP） internal fixation;25 pa- tients, group C,were treated by external fixator. All cases were followed up regularly. Johner-Wruhs criteria was used to analyze the function of ankle-knee joint. Results 75 cases were followed up for 10 - 36 （ 25 ± 1.5 ） months. The function recovery of knee and ankle joint, its partial weight-bearing time, and tibia fracture healing time in group A and group B was better than group C. The three therapeutic effects were 94. 6%, 94. 0% and 81.4%. Conclusions The efficacy of interlocking intramedullary nail and LCP in the treatment of Gustilo Ⅱ shaft of tibial fractures is sat- isfied.
