摘要:目的:观察参附注射液对大鼠胆道缺血再灌注的保护作用及其对细胞凋亡的影响,并探讨其中可能的保护机制。方法:60只清洁级SD雄性大鼠随机分为六组,每组10只,分别为:假手术对照组、缺血再灌注组、参附注射液组(15 m L/kg组、10 m L/kg组、5 m L/kg组)和依达拉奉组。术后4 h采取血清及肝组织,应用免疫组化SP法检测胆管组织TNF-a、Fas的表达,光镜下观察肝脏病理学改变;测定丙二醛(MDA)含量、超氧化物歧化酶(SOD)活性;测定血清谷丙转氨酶(ALT)、碱性磷酸酶(ALP)、总胆红素(TBIL)。结果:与假手术对照组比较,缺血再灌注组ALT、ALP、TBIL水平显著升高,胆管组织SOD活性明显升高,MDA含量明显降低,病理学结果显示胆管组织损伤明显,TNF-a、Fas表达明显增强。与缺血再灌注组比较,参附注射液15 m L/kg组、10 m L/kg组均能降低血清ALT、ALP、TBIL水平及胆管组织MDA含量,升高SOD活性;参附注射液5 m L/kg组能降低血清ALT水平,升高SOD活性。病理学结果显示参附各剂量组胆管组织损伤明显减轻,TNF-a、Fas表达下降显著。结论:参附注射液预处理能够抑制TNF-a/TNF-R1→Fas/Fas-L死亡信号转导途径,拮抗胆管上皮细胞的凋亡,改善胆管功能,有效降低肝移植术后胆道并发症。
Objective To study the effect of Shenfu injection on ischemia reperfusion injury (IRI)and on the apoptosis of bile duet in rats,and explore its possible protective mechanism.Methods 60 SD male rats were randomly divided into 6 group (n =10 in each group):Sham group,IR group,Shenfu dose groups (15.10.5 mL/kg)and edaravone group.Shenfu dose groups were re- ceived intraperitoneal Injection of Shenfu (15.10.5 mL/kg/d,ip)at the same time every day for 3 d;Blood and hepatic tissus Sample were harvested at 4h after reperfusion.Expression of TNF -a,Fas were examined and content of SOD.MDA.ALT.ALP and TBIL were Measured.Result Compared with Sham group,the levels of ALT,AST,TBIL in serum and the contents of MDA were significantly increased,the activity of SOD in bile duet tissue were decreased in IR group.Pathological results showed that the bile duet tissue injury was more obvious in IR group.The expression of TNF-α and Fas were increased obviously,compared with the IR group,the levels of ALT,AST,TBIL in serum and the eontents of MDA in bile duct tissue were decreased while the activity of SOD were increased in SF dose groups (15,10 mL/kg);The levels of ALT in serum were decreased while the activity of SOD were increased in SF dose group (5 mL/kg),The pathological changes of bile duet injury could be improved in SF each dose group (15 10,5 mL/kg),The expression of TNF-α and Fas were decreased obviously.Conclusion Shenfu injection can inhibits expressions of TNF-α and Fas,regulate the apoptosis of bile duct cells,improve bile duct function and reduce biliary traet complications after liver transplantation.
