摘要：高血压病是一种进行性的"心血管综合征",也是心脑血管病最主要的危险因素。现代医学多认为高血压病是遗传因素和环境因素相互作用的结果,而最新研究表明肠道菌群紊乱与高血压病密切相关。尽管药物治疗和生活方式干预已被广泛应用,但高血压病发病率仍在持续上升,长期血压升高所带来的靶器官损害严重影响患者生存和预后。高血压病根据其临床表现及特点属中医"眩晕""头痛"等范畴,古今医家大多从肝肾,或从痰浊,或从瘀血来认识和治疗此病,往往对"后天之本"脾胃在本病中的作用没有给予足够的重视及研究。该文通过分析"培土宁风"中医传统理论内涵和现代医学视角下的相关认识,认为土虚风动为高血压病基本病因病机,病变脏腑以肝脾为主,久则及肾,风邪、痰浊、瘀血贯穿疾病发生发展的全过程,并联系半夏白术天麻汤方药特点及名医经验探讨培土宁风法治疗高血压病之机理,指出在培土宁风思想指导下,以调理脾胃为切入点,改善肠道微生态,可能是治疗高血压病的有效措施,以期为临床有效防治此病提供新途径。
Hypertension is a kind of progressive“cardiovascular syndrome”and it is also the most important risk factor for cardio-cerebrovascular disease.Modern medicine thinks that hypertension is the result of the interaction of genetic factors and environmental factors,but the latest research shows that the disorder of intestinal flora is closely related to hypertension.Although drug therapy and lifestyle intervention have been widely used,the incidence of hypertension continues to rise,and the target organ damage caused by long-term elevated blood pressure seriously affects the survival and prognosis of patients.According to its clinical manifestations and characteristics,hypertension belongs to the category of“vertigo”and“headache”in traditional Chinese medicine.Ancient and modern doctors mostly recognize and treat the disease from liver and kidney,turbid-phlegm or blood stasis.The role of spleen and stomach in this disease has not been given enough attention and research.Based on the analysis of the connotation of traditional Chinese medicine theory of“cultivating earth and calming wind”method and the relevant understanding from the perspective of modern medicine,this paper holds that the basic etiology and pathogenesis of hypertension is earth deficiency and wind movement,and the diseased viscera are mainly liver and spleen,and then the kidney is involved for a long time.Wind evil,phlegm-turbidity and blood stasis run through the whole process of the occurrence and development of the disease,and the mechanism of cultivating earth and calming wind method in the treatment of hypertension was discussed in connection with the characteristics of Banxia Baizhu Tianma Decoction(半夏白术天麻汤)and the experience of famous doctors.It is pointed out that under the guidance of the thought of cultivating earth and calming wind method and taking the recuperation of spleen and stomach as the starting point,improving intestinal microecology may be an effective measure for the treatment of hypertension,in order to provide a new way for clinical prevention and treatment of this disease.
