摘要：近年来,干眼症的发病率逐年攀升,已成为最常见的眼科疾患之一。临床类型呈多样化发展,临床可见颈椎病患者多伴随干眼症相关临床表现,如双眼干涩不适、痒痛、畏光伴颈部疼痛、上肢麻木等。就治疗言,多局限于取标穴,而忽视本穴,轻视脏腑精气的整体调节功能。《伤寒论》云:"观其脉证,知犯何逆,随证治之",故要做到标本兼顾,又要重视脏腑精气流注、濡养之效及其相关生理、病理变化。就其生理言,脏腑精气充沛,运行通畅,则双目得养,眼表长期得滋;就病理言,颈部精气不足、经气不利,无以上荣五官清窍,双目失濡,则发为干眼。干眼症不仅有眼表的症状,更多的是伴有其他症状,如颈部相关症状,此最易忽视,故干眼症的治疗不能拘泥于局部取穴。据此,文章基于《灵枢·大惑论》可见"五脏六腑之精气,皆上注于目而为之精……后出于项中"的理论,知脏腑精气直接联系双眼与颈项,通过颈项部经脉循行将津液上输至眼周,从而达濡养双目之效,若颈项枢机不利,脏腑精气失于上荣,则发干眼,进一步提出"从颈治眼"理论,并阐述经络相关机制。基于此,文章从生理基础、经脉、经别、经筋、特效穴位等几个方面对针灸治疗干眼症的诊疗思路进行探讨,以期为治疗本病提供理论依据及新的思路。
In recent years, the incidence of dry eye has been increasing year by year, and it has become one of the most common eye diseases.The clinical types are diversified. Clinically, patients with cervical spondylosis are often accompanied by clinical manifestations related to dry eye syndrome, such as dry and uncomfortable eyes, itching, photophobia with neck pain, and upper limb numbness. In terms of treatment, most of them are limited to taking standard points, but ignore their own points and despise the whole regulating function of Zang-Fu organs essence and Qi.Treatise on Febrile Diseases says: "Look at the syndrome of the pulse, know what is wrong, and treat it with the syndrome." Therefore, it is necessary to take into account the specimens, and pay attention to the effect of Zang-Fu organsl sperm injection, maintenance and related physiological and pathological changes. In terms of the physiology, the organs are full of vital energy and smooth running, and the eyes are nourished, and the eyes are nourished for a long time. In terms of pathology, the neck has insufficient Qi and poor menstrual Qi, the eyes are lack of nourishing and become dry. Dry eye syndrome not only has symptoms on the ocular surface, but also is accompanied by other symptoms, such as neck-related symptoms, which are most easily overlooked. Therefore, the treatment of dry eye syndrome cannot be restricted to local acupoint selection. Based on the theory of "the essence and Qi of five Zang organs and six Fu organs are all injected into the eye and then out of the neck" in Lingshu·Dahuolun, this paper points out that the essence and Qi of Zang organs and six Fu organs are directly connected with the eyes and neck, and the body fluid is transported to the eye through the meridians of the neck, so as to nourish the eyes, Furthermore, the theory of "treating eye from neck" was put forward, and the related mechanism of meridians was expounded. Based on this, this paper discussed the diagnosis and treatment of dry eye treated by acupuncture from the aspects of physiological basis, meridians, meridians, tendons and special acupoints, in order to provide theoretical basis and new ideas for the treatment of this disease.
