摘要:目的:基于“病-证-症-方”关联网络分析策略,探析杜仲腰痛丸干预腰椎间盘突出症(LDH)的功效特点和作用机制。方法:根据杜仲腰痛丸组方方解对功效组进行划分;通过网络药理学相关数据库获取杜仲腰痛丸活性成分及相关靶点预测,构建蛋白质互作网络,对关键靶点进行生物富集分析。使用斯泼累格·多雷雄性大鼠进行LDH模型制备;利用转录组学、中医证候本体及多维定量关联计算平台及基因集分析杜仲腰痛丸干预LDH机制;采用实时荧光定量PCR检测信号通路基因mRNA水平变化。结果:网络分析表明,杜仲腰痛丸方中补肾壮腰组、活血消肿组与通络止痛组的药理特点分别概括为神经功能改善、炎症反应调节、激素代谢水平纠正;结合转录组学、基因集富集分析及分子生物学结果,发现杜仲腰痛丸抑制磷脂酰肌醇3-激酶(PI3K)、蛋白激酶B(AKT)基因表达(P<0.01),干预炎症介质释放如白细胞介素-8(IL-8)(P<0.05)、肿瘤坏死因子-α(TNF-α)(P<0.01),通过纠正疾病发生发展中的“神经-免疫-内分泌”失衡系统来干预LDH关键致病基因。结论:杜仲腰痛丸通过调节神经功能、激素水平及改善炎症反应,发挥温肾壮腰、行气止痛的功效,从而缓解LDH疼痛症状。
Objective:Based on the“disease-syndrome-symptom-formula”network analysis strategy,this study aimed to investigate the pharmacological characteristics and mechanisms of Duzhong Yaotong Pills in the treatment of lumbar disc herniation(LDH).Methods:Efficacy groups were categorized according to the formula composition and traditional explanations of Duzhong Yaotong Pills.Active ingredients and related targets were identified using network pharmacology databases,and protein-protein interaction networks were constructed.Key targets were subjected to bioinformatic enrichment analysis.Sprague Dawley male rats were used to establish an LDH model.Transcriptomics,the Traditional Chinese Medicine Syndrome Ontology,multi-dimensional quantitative association analysis platforms,and gene set analysis were applied to explore the mechanisms of Duzhong Yaotong Pills in LDH.Real-time quantitative PCR was used to detect changes in mRNA expression of signaling pathway genes.Results:Network analysis showed that the Bushen Zhuangyao group,Huoxue Xiaozhong group,and Tongluo Zhitong group of Duzhong Yaotong Pills were primarily associated with improving neurological function,regulating inflammatory responses,and correcting hormone metabolism levels,respectively.Integrating transcriptomics,gene set enrichment analysis,and molecular biology results,Duzhong Yaotong Pills were found to inhibit the expression of phosphoinositide 3-kinase(PI3K)and protein kinase B(AKT)genes(P<0.01),modulate the release of inflammatory mediators such as interleukin-8(IL-8)(P<0.05)and tumor necrosis factor-α(TNF-α)(P<0.01),and intervene in key pathogenic genes of LDH by correcting imbalances in the neuro-immune-endocrine system during disease progression.Conclusion:Duzhong Yaotong Pills alleviate LDH-related pain by regulating neurological function,hormone levels,and inflammatory responses,thereby exerting therapeutic effects consistent with warming the kidneys,strengthening the waist,promoting qi circulation,and relieving pain.
