摘要:目的:研究景芪愈溃胶囊对脾胃虚寒型胃溃疡大鼠的影响并探讨其机制。方法:将70只SPF级Wistar大鼠按完全随机设计分为空白组和造模组,造模组采用大黄苦寒泻下法+饥饱失常喂养法+改良Okabe法综合因素构建脾胃虚寒型胃溃疡大鼠模型,造模成功后按随机数字表法分为模型组,景芪愈溃胶囊高、中、低剂量组,中药阳性对照(安胃疡胶囊)组,以及西药阳性对照(雷贝拉唑钠肠溶胶囊)组。空白组和模型组给予2 mL/d蒸馏水灌胃,其余各药物干预组分别给予对应剂量的胶囊内容物混悬液2 mL/d灌胃,持续干预14 d。使用游标卡尺测量胃溃疡最大长径并计算溃疡指数(UI);采用HE染色观察大鼠胃组织形态学变化;采用ELISA法检测大鼠胃组织中白细胞介素1(IL-1)、IL-4、IL-6和IL-13的含量;采用RT-qPCR法检测大鼠胃组织中血管细胞黏附因子1(VCAM-1)和细胞间黏附分子1(ICAM-1)的mRNA表达水平,并采用Western blot法检测大鼠胃组织中上述指标的蛋白相对表达水平。结果:与空白组比较,模型组大鼠胃黏膜可见明显溃疡面,光镜下观察可见黏膜组织缺失和坏死,胃组织IL-1和IL-6含量明显升高,IL-4和IL-13含量明显降低,VCAM-1和ICAM-1的mRNA和蛋白表达水平显著升高(P<0.01);与模型组比较,景芪愈溃胶囊高剂量组大鼠的UI显著降低,光镜下观察胃溃疡的恢复情况最接近空白组,胃组织IL-1和IL-6含量显著降低,IL-4和IL-13含量显著升高,VCAM-1和ICAM-1的mRNA和蛋白表达水平显著降低(P<0.01)。结论:景芪愈溃胶囊可有效促进胃溃疡的愈合,其机制可能与抑制细胞黏附分子VCAM-1和ICAM-1表达进而减少促炎因子IL-1和IL-6含量,增加抗炎因子IL-4和IL-13含量,从而调节促炎因子与抗炎因子的平衡有关。
AIM:To discuss the effects of Jingqi-Yukui capsule(JQYK)on rats with gastric ulcer of spleenstomach deficient cold type and the mechanisms.METHODS:Specific pathogen-free Wistar rats(n=70)were divided into blank group and model group according to complete random design.A rat model of gastric ulcer of spleen-stomach deficient cold type was established in model group by combined factors,including purgation with drugs of bitter flavour and cold nature,feeding method of abnormal starvation and full,and improved Okabe method.After successful modeling,the rats were divided into model group,high-,medium-and low-dose JQYK groups,traditional Chinese medicine positive control(An-Weiyang capsule)group and western medicine positive control(rabeprazole sodium enteric-coated capsule)group by random digital table.The rats in blank group and model group were given 2 mL/d of distilled water by gavage,while those in rest drug intervention groups were given 2 mL/d of suspension by gavage,which was prepared with the contents of corresponding dosage of drug capsule and distilled water.All rats were continuously intervened for 14 d.The maximum length of gastric ulcer was measured using vernier caliper,and ulcer index(UI)was calculated.HE staining was used to observe the morphological changes of gastric tissues in the rats.The levels of interleukin-1(IL-1),IL-4,IL-6 and IL-13 in rat gastric tissues were measured by ELISA.The mRNA and protein levels of vascular cell adhesion molecule-1(VCAM-1)and intercellular adhesion molecule-1(ICAM-1)in rat gastric tissues were determined by RT-qPCR and Western blot.RESULTS:Compared with blank group,the gastric tissue of the rats in model group showed significant ulcer surface by visual inspection,and loss and necrosis were observed by light microscopy.The levels of IL-1 and IL-6 in gastric tissue of the rats in model group were significantly increased,while IL-4 and IL-13 were significantly decreased.The mRNA and protein expression levels of VCAM-1 and ICAM-1 in gastric tissue of the rats in model group were significantly increased(P<0.01).Compared with model group,UI was significantly reduced in high-dose JQYK group,and the recovery of gastric ulcer under light microscope was the closest to that in blank group.The levels of IL-1 and IL-6 in gastric tissue of the rats in high-dose JQYK group were significantly decreased,while IL-4 and IL-13 were significantly increased.The mRNA and protein expression levels of VCAM-1 and ICAM-1 in gastric tissue of the rats in high-dose JQYK group were significantly decreased(P<0.01).CONCLUSION:Jingqi-Yukui capsule effectively promotes the healing of gastric ulcer.Its mechanism may be related to inhibiting the expression of cell adhesion molecules VCAM-1 and ICAM-1,reducing the content of pro-inflammatory factors IL-1 and IL-6 and increasing the content of anti-inflammatory factors IL-4 and IL-13,thus regulating the balance of pro-inflammatory factors and anti-inflammatory factors.
