摘要：霰粒肿为眼睑常见病,其发病因素和病程中西医观点不同,治疗方法多样,中医治疗霰粒肿临床疗效显著,安全且复发率低,患者接受程度较高。本研究通过总结近年霰粒肿的相关文献,进一步明确霰粒肿中西医病因病机,对各类治疗方法进行分类总结,为后续治疗本病寻求理论依据和方法支持。利用中国知网、万方数据库和PubMed等中英文数据库,以"霰粒肿""睑板腺囊肿"和"胞生痰核"等为关键词检索2010-01-01-2019-02-20相关文献。纳入符合标准的文献60余篇,剔除综述类及部分质量较差的文献,最终29篇文献纳入分析。西医认为,霰粒肿发病可能与睑板腺分泌旺盛、炎症反应和感染等多种因素密切相关;其病程发展较缓慢可合并感染;主要治疗方法包括保守治疗、局部技术注射和手术等。中医认为,霰粒肿的发生发展与脏腑相关,包括脾胃二经蕴热循经上犯于胞睑、痰湿痰热凝结积聚于胞睑和肝经之热达之胞睑等。霰粒肿中医特色治疗历史悠久、方法多样,包括内服中药、针灸、中医外治和中西医结合治疗,不仅可以消除硬结,而且可以降低霰粒肿的复发率,具有良好的临床疗效和患者依从性。但中医治疗缺乏统一的治疗规范及标准,未来研究方向应该致力于筛选出真实有效的治疗方法,并推广应用于临床治疗。
As a common disease of eyelid,chalazion is characterized by different pathogenic factors and views of traditional Chinese and western medicine on the course of the disease,and various treatment methods.The clinical efficacy of TCM treatment for chalazion is significant,safe,low recurrence rate and high acceptance level of patients.By summarizing the literature in recent years,the review is to further clarify the causes of Chinese and Western medicine for chalazion,classify and summarize various treatment methods,and seek theoretical basis and method support for the follow-up treatment of this disease.By using Chinese and English databases such as China National Knowledge Infrastructure(CNKI),Wanfang Data,and PubMed.The relevant literature was searched for keywords such as"chalazion""eyelid cyst""baoshengtanhe"from January 1,2010 to February 20,2019.More than 60 documents were collected meeting the criteria,then the review articles and some poor quality documents among them were excluded.The final 29 articles were included in the analysis.Western medicine believes that the chalazion which develops slowly may be closely related to the over-secretion of meibomian glands,inflammation,infection and other factors.The main treatment methods include conservative treatment,local intralesional injections,and surgery.According to traditional Chinese medicine,the occurrence and development of graupel granuloma are related to viscera,including spleen and stomach,phlegm,dampness and phlegm,and liver meridian.Characteristic treatment of chalazion by Chinese medicine has a long history and a variety of methods,including internal Chinese medicine,acupuncture,external treatment of Chinese medicine and integrated Chinese and western medicine treatment,which can not only eliminate the sclerosis,but also reduce the recurrence rate of chalazion,with good clinical efficacy and patient compliance.However,there is no unified treatment standard and standard for traditional Chinese medicine treatment,so the future research direction should be devoted to screening out real and effective treatment methods and promoting them to a larger range of clinical treatment.
