摘要：慢性腰痛已日渐成为突出的公共卫生问题,椎间盘退变是其主要致病原因,细胞外基质降解、细胞凋亡、细胞焦亡及氧化应激等是椎间盘退变发生发展的关键机制。肿瘤坏死因子-α(TNF-α)作为核心促炎因子,在椎间盘退变进程中发挥关键作用,其可通过加速细胞外基质降解、激活氧化应激通路、调控相关信号通路及非编码RNA表达加剧细胞功能紊乱,还能诱发炎症级联反应形成恶性循环,进一步推动椎间盘退变的发展。目前临床保守治疗与手术干预均存在一定的局限性,而中医药凭借其多成分、多靶点的独特优势,在椎间盘退变治疗中展现出良好潜力。本文系统综述了TNF-α介导椎间盘退变的核心机制,详细阐述了中医药的干预策略,涵盖中药单体、中药复方以及针灸、推拿等物理疗法通过调节免疫微环境实现防治椎间盘退变的治疗价值,以期为椎间盘退变的中医药防治提供全面的理论参考与新的治疗思路。
Chronic low back pain has become a prominent public health problem.Intervertebral disc degeneration is the main cause of chronic low back pain.Extracellular matrix degradation,apoptosis,pyroptosis and oxidative stress are the key mechanisms for the occurrence and development of intervertebral disc degeneration.As a core pro-inflammatory factor,tumor necrosis factor-α(TNF-α)plays a key role in the process of intervertebral disc degeneration.It can aggravate cell dysfunction by accelerating extracellular matrix degradation,activating oxidative stress pathways,regulating related signaling pathways and non-coding RNA expression.It can also induce a vicious cycle of inflammatory cascades to further promote the development of intervertebral disc degeneration.At present,there are some limitations in clinical conservative treatment and surgical intervention,and traditional Chinese medicine has shown good potential in the treatment of intervertebral disc degeneration with its unique advantages of multi-component and multi-target.This article systematically reviews the core mechanism of TNF-α-mediated intervertebral disc degeneration,and elaborates on the intervention strategies of traditional Chinese medicine,covering the therapeutic value of traditional Chinese medicine monomer,traditional Chinese medicine compound,acupuncture,massage and other physical therapies in preventing and treating intervertebral disc degeneration by regulating the immune microenvironment.The aim is to provide a comprehensive theoretical reference and new treatment ideas for the prevention and treatment of intervertebral disc degeneration with traditional Chinese medicine.
