摘要:目的:比较刃针松解筋结点与常规针刺治疗瘀阻脑络型颈源性头痛的临床疗效差异。方法:将78例患者随机分为刃针松解组和常规针刺组各39例,刃针松解组以筋结点为治疗部位进行刃针操作,1次/3 d,两次/周,共治疗两周。常规针刺组以常规经穴为主,1次/d, 5次/周,共治疗两周。观察两组治疗前后疼痛视觉模拟评分(VAS)、颈椎功能障碍指数量表(NDI)和中医证候总积分的变化,评估两组疗效。结果:治疗后两组VAS评分、NDI评分和中医证候总积分较治疗前均降低,差异有统计学意义(P<0.05),刃针松解组的评分较常规针刺组降低更明显,差异有统计学意义(P<0.05);刃针松解组总有效率为94.59%(35/37),明显高于常规针刺组的76.32%(29/38),两组疗效差异具有统计学意义(P<0.05)。结论:刃针松解筋结点治疗颈源性头痛疗效显著,能明显缓解疼痛及改善颈椎功能活动障碍。
Objective: To compare the difference of clinical efficacy between blade needle loosening tendon nodes and conventional acupuncture in the treatment of cervicogenic headache caused by blood stasis obstructing cervical collaterals. Methods: 78 patients were randomly divided into the blade needle loosening group and the conventional acupuncture group, with 39 cases in each group. In the blade needle loosening group, the tendon nodes were used as the treatment site to perform the blade needling, once every 3 days, twice a week, for a total of 2 weeks. In the conventional acupuncture group, the conventional acupoints were mainly chosen for needling, once a day, 5 times a week, for a total of 2 weeks. The scores of visual analogue scale of pain(VAS), the neck dysfunction index(NDI), and the total score of TCM syndromes were observed before and after the treatment in the two groups. The clinical efficacy was compared between the two groups. Results: VAS score, NDI score and the total score of TCM syndromes were all lower after the treatment than those before the treatment in the two groups(P<0.05);of which the improvements were more significant in the blade needle loosening group than those in the conventional acupuncture group(P<0.05). The total effective rate of the blade needle loosening group was 94.59%(35/37), which was significantly higher than 76.32%(29/38) of the conventional acupuncture group(P<0.05). Conclusion: Blade needle loosening tendon nodes is effective in treating cervicogenic headache, which can significantly relieve pain and improve cervical spine dysfunction.
