摘要:目的探讨基质金属蛋白酶-3(MMP-3)基因启动子-1171(5A/6A)多态性对高血压患者左室重构的影响。方法采用限制性片段长度多态性分析技术,检测104例高血压病患者和36例对照者的MMP-3基因启动子-1171(5A/6A)基因型的表达,并且对并且对104例高血压患者和46例正常人进行超声心动图检查,计算左室重量(LVM)及左室重量指数(LVMI)。结果高血压病组与对照组MMP-3基因启动子-1171(5A/6A)基因型分布比较差异无统计学意义。高血压病组的LVM、LVMI水平与对照组比较,差异均有统计学意义(P〈0.05)。高血压病组内6A/6A基因型者的LVM、LVMI水平较5A/6A和5A/5A型高血压病患者升高,且两组女性患者LVMI水平比较差异有统计学意义(P〈0.05)。多因素方差分析及多元逐步回归分析显示,LVMI与患者高血压病程、脉压、6A/6A基因型等均有不同程度的相关。结论MMP-3基因启动子-1171(5A/6A)多态性对高血压病患者左室重构有影响。
Objective To investigate the possible impact of the polymorphism in 5A/6A Matrix metalloproteinase-3 (MMP-3) gene promoter on the risk of left ventricle hypertrophy Methods 104 hy- pertensives and 36 control subjects were recruited. Polymerase chain reaction followed by restrictionfrag- ment length polymorphism(PCR-RFLP) was used to detect SNP-1171 (5A/6A)in MMP-3 gene promoter region. LV mass (LVM) was calculated using theformula : 0. 8 × ( 1.04 × [ ( LVIDd + IVSd + PWd) 3 _ (LVIDd) 3 ) ] + 0.6. Left ventricular mass index (LVMI) was determined using formula: LVM/BSA, where BSA was the body surface area. Results The distribution of MMP-3 genotype was not significantly differ- ent between hypertensive patients and controls. LVMI were signnieantly higher in the subjects than con- trois. Genotype 6A/6A in hypertensives LVMI higher than 5A/6A and 5A/5A though males and females. When Univariate Stepwise Regression analysis was used, we found there increased risk for left ventricular remodling associated with course, pulse pressure and genotype 6A/6A in hypertensive patients than con- trois. Coneinsion These results indicate that there was differences in association of MMP-3 5A/6A poly- morphism with left ventrieular hypertrophy.
