摘要：目的:运用Meta分析的方法对黄芪建中汤治疗脾胃虚寒型胃溃疡的临床有效性与安全性进行评价。方法:计算机检索PubMed、Cochrane Library、Web of Science、Embase、中国知网、中国生物医学文献、维普、万方等八大数据库中黄芪建中汤治疗脾胃虚寒型胃溃疡临床疗效的相关文献,检索时限为建库至2020年7月,纳入黄芪建中汤治疗脾胃虚寒型胃溃疡的临床随机对照试验,按照Cochrane系统评价偏倚风险评估工具对文章的风险进行评价,采用Review Manager 5.3软件进行数据分析。结果:共纳入12篇临床随机对照试验文献,共1499例患者。Meta分析结果显示:①试验组的总有效率优于对照组[RR=1.12,95%CI(1.08,1.17),P<0.00001];②试验组幽门螺杆菌(Helicobacter pylori,Hp)转阴率高于对照组[RR=1.17,95%CI(1.07,1.28),P=0.0003];③治疗后试验组复发率低于对照组[RR=0.25,95%CI(0.18,0.35),P<0.00001];④试验组不良反应发生率与对照组无明显差异[RR=0.47,95%CI(0.06,3.87),P=0.48]。结论:黄芪建中汤治疗脾胃阳虚型胃溃疡具有更好的疗效,能够增加Hp的转阴率并且降低复发率。
Objective:To evaluate the clinical effectiveness and safety of the Huangqi Jianzhong decoction(黄芪建中汤)on gastric ulcer with spleen-stomach deficiency cold type by meta analysis.Methods:In eight databases including PubMed,Cochrane Library,Web of Science,Embase,CNKI,Chinese Biomedical Literature,VIP,Wanfang Data,etc.,the relevant literature on gastric ulcer with spleen-stomach deficiency cold type was retrieved,the publish time is from the establishment of the database to July 2020.The randomized controlled trials of the Huangqi Jianzhong decoction on gastric ulcer with spleen-stomach deficiency cold type were included,and the risk of the literature was evaluated according to the Cochrane systematic evaluation bias risk assessment tool.Statistical analysis was performed using RevMan 5.3 software.Results:A total of 12 RCTs literatures were included,with a total of 1499 patients.Meta analysis results show:①The total effective rate in the experimental group is better than that in the control group[RR=1.12,95%CI(1.08,1.17),P<0.00001].②In the experimental group,the rate of Hp becoming negative is higher than the control group[RR=1.17,95%CI(1.07,1.28),P=0.0003].③The recurrence rate in the experimental group after treatment is lower[RR=0.25,95%CI(0.18,0.35),P<0.00001].④There was no significant difference in the incidence of adverse reactions between the experimental group and the control group[RR=0.47,95%CI(0.06,3.87),P=0.48].Conclusion:The Huangqi Jianzhong decoction has a better curative effect in treating gastric ulcer with spleen-stomach deficiency cold type,increasing the rate of Hp becoming negative and reducing the recurrence.
