摘要：目的观察骨痛灵酊联合电针辅助治疗颞下颌关节紊乱(TMD)寒湿痹阻证的临床疗效。方法将86例TMD寒湿痹阻证患者采用随机数字表法分为对照组和治疗组,各43例。2组均给予一般健康宣教,对照组在此基础上给予西药常规治疗;治疗组在对照组治疗方法的基础上给予骨痛灵酊联合电针辅助治疗,2组均以7d为1个疗程,连续治疗2个疗程后比较2组的临床疗效及治疗前后最大张口度、视觉模拟评分法(VAS)评分、Helkimo指数及颞下颌关节功能障碍(DI)指数。结果对照组总有效率为79.1%(34/43),治疗组为95.3%(41/43),2组比较差异有统计学意义(P<0.01)。治疗后2组最大张口度明显增大,VAS评分、Helkimo指数、DI指数明显降低,与同组治疗前比较差异均有统计学意义(P<0.01),且治疗组各项指标改善更明显,与对照组治疗后比较差异均有统计学意义(P<0.01)。结论骨痛灵酊联合电针辅助治疗TMD寒湿痹阻证临床疗效显著,可有效缓解患者疼痛,改善颞下颌关节功能,提高患者生活质量,值得临床推广应用。
ObjectiveTo observe theclinical efficacyofGutongling Tincture(骨痛灵酊)combined with electroacupuncture as adjuvant therapy for temporomandibular disorders(TMD)with cold-dampness obstruction syndrome.Methods A total of 86 patients with TMD and cold-dampness obstruction syndrome were randomly divided into a control group and a treatment group using a random number table,with 43 cases in each group.Both groups received general health education.The control group was additionally treated with conventional Western medicine,while the treatment group received Gutongling Tincture combined with electroacupuncture in addition to the control group's treatment regimen.Both groups underwent a 7-day treatment course,and after two consecutive courses,clinical efficacy was compared.Parameters evaluated included maximum mouth opening,Visual Analogue Scale(VAS)scores,Helkimo index,and temporomandibular dysfunction(DI)index before and after treatment.Results The total effective rate was 79.1%(34/43)in the control group and 95.3%(41/43)in the treatment group,with a statistically significant difference between the two groups(P<0.01).After treatment,both groups showed significant increases in maximum mouth opening and significant reductions in VAS scores,Helkimo index,and DI index compared to pre-treatment levels(P<0.01).Moreover,the improvement in all indicators was more pronounced in the treatment group,with statistically significant differences compared to the control group after treatment(P<0.01).Conclusion Gutongling Tincture combined with electroacupuncture as adjuvant therapy demonstrates significant clinical efficacy in treating TMD with cold-dampness obstruction syndrome.It effectively alleviates pain,improves temporomandibular joint function,and enhances patients'quality of life,making it worthy of clinical promotion and application.
