摘要:目的从临床使用的动脉血气、超敏C反应蛋白(hs-CRP)、N末端B型钠尿肽前体(NT-proBNP)及超声心动图等简便、易得的检查中发现与慢性肺源性心脏病病情严重程度相关的指标。方法选取2016年1月至2019年1月甘肃中医药大学附属医院老年医学科的96例慢性肺源性心脏病住院患者,根据诊断标准及是否出现并发症分为代偿期组(n=36)及失代偿期组(n=60),比较两组间年龄、pH值、二氧化碳分压(PCO2)、氧分压(PO2)、hs-CRP、NT-proBNP及右心房长径、右心房横径、右心室前后径、右心室长径、右心室横径、右心室流出道内径、肺动脉内径、肺动脉收缩压等指标差异,并采用Logistic回归分析与慢性肺源性心脏病病情严重程度相关的因素。结果两组pH值、PCO2、PO2比较,差异无统计学意义(P>0.05);失代偿期组hs-CRP、NT-proBNP、右心房长径、右心房横径、右心室前后径、右心室长径、右心室横径、右心室流出道内径、肺动脉内径等指标均高于代偿期组,差异有统计学意义(P<0.05);Logistic回归分析提示,慢性肺源性心脏病病情严重程度的影响因素包括年龄、hs-CRP、NT-proBNP。结论年龄、hs-CRP、NT-proBNP与慢性肺源性心脏病病情严重程度相关。
Objective To discover the indexes related to the severity of chronic pulmonary heart disease from the simple and easy examinations such as arterial blood gas,high-sensitivity C-reactive protein(hs-CRP),N-terminal B-type natriuretic peptide(NT-proBNP)and echocardiography.Methods A total of 96 patients with chronic pulmonary heart disease admitted to the department of geriatrics in Affiliated Hospital of Gansu University of Chinese Medicine from January 2016 to January 2019 were retrospectively collected.They were divided into the compensatory period group(n=36)and the decompensated period group(n=60)according to the diagnostic criteria and complications.The differences of age,pH value,partial pressure of carbon dioxide(PCO2),partial pressure of oxygen(PO2),hs-CRP,NT-proBNP,right atrial long diameter,right atrial transverse diameter,right ventricular anteroposterior diameter,right ventricular long diameter,right ventricular transverse diameter,right ventricular outflow tract diameter,pulmonary artery diameter and pulmonary artery systolic pressure between the two groups were compared.Logistic regression analysis was used to analyze the factors related to the severity of chronic pulmonary heart disease.Results There was no significant difference in pH value and PCO2、PO2 between the two groups(P>0.05).hs-CRP,NT-proBNP,right atrial long diameter,right atrial transverse diameter,right ventricular anteroposterior diameter,right ventricular long diameter,right ventricular transverse diameter,right ventricular outflow tract diameter,pulmonary artery diameter in the decompensated group were higher than those in the compensated group,with statistically significant differences(all P<0.05).Logistic regression analysis showed that the impacting factors of the severity of chronic pulmonary heart disease include age,hs-CRP and NT-proBNP.Conclusion Age,hs-CRP and NT-proBNP are correlated with the severity of chronic pulmonary heart disease.
