摘要:目的观察三黄凝胶对大鼠耳郭痤疮模型血清白细胞介素-17(IL-17)、白细胞介素-23(IL-23)水平的影响,探究外涂三黄凝胶治疗痤疮的作用机制。方法从50只Wistar大鼠中随机选取8只作为空白组,其余大鼠采用煤焦油涂抹于右耳郭耳导管开口处方法建立耳郭痤疮模型,将造模成功的40只大鼠随机分为模型组、维A酸乳膏组、三黄凝胶低剂量组、三黄凝胶中剂量组、三黄凝胶高剂量组,每组8只。模型组不涂抹任何药物,维A酸乳膏组造模处涂抹维A酸乳膏,三黄凝胶各组造模处涂抹相应剂量的三黄凝胶,连续干预4周。造模和干预期间观察各组大鼠耳部皮损变化;末次用药24 h后大鼠心脏取血,通过ELISA法检测血清IL-17、IL-23水平;处死各组大鼠,取患处耳部组织,HE染色镜下观察。结果各造模组大鼠造模结束后,耳郭肿胀,皮温升高,表皮增厚且质地粗糙,有较严重的脱屑、痂皮症状,毛囊孔增大。干预结束后,三黄凝胶低剂量组大鼠仍有较多未治愈粉刺;三黄凝胶中剂量组和维A酸乳膏组大鼠毛囊角栓和结节显著减少,但仍有少量脱屑;三黄凝胶高剂量组大鼠耳皮肤无肿胀、脱屑及痂皮现象,基本恢复正常。模型组大鼠血清IL-17、IL-23水平均明显高于空白组(P均<0.05);各干预组大鼠血清IL-17、IL-23水平均明显低于模型组(P均<0.05),且三黄凝胶高剂量组均明显低于维A酸乳膏组(P均<0.05),三黄凝胶低剂量组、中剂量组与维A酸乳膏组比较差异均无统计学意义(P均>0.05)。光镜下三黄凝胶低剂量组毛囊口扩张程度有所减轻,炎性细胞较模型组少;三黄凝胶中剂量组和维A酸乳膏组可见毛囊口扩张程度减轻,炎性细胞减少;三黄凝胶高剂量组炎性细胞显著减少。结论三黄凝胶可有效治疗痤疮,机制可能与其降低IL-17、IL-23表达有关。
Objective It is to observe the effect of Sanhuang gel on the levels of serum interleukin-17(IL-17)and interleukin-23(IL-23)in rat models with auricular acne,and to explore the mechanism of external application of Sanhuang gel in the treatment with auricular acne.Methods Eight Wistar rats were randomly selected as the blank group from 50 Wistar rats.The remaining rats were smeared with coal tar on the opening of ear catheter of the right ear to establish auricular acne models.The 40 rats that were successfully modeled were randomly divided into model group,tretinoin cream group,low-dose Sanhuang gel group,medium-dose Sanhuang gel group,high-dose Sanhuang gel group,8 animals in each group.No drugs were applied to the model group,tretinoin cream was applied to the modeling place of the tretinoin cream group,and the corresponding dose of Sanhuang gel was applied to the modeling place of each group of Sanhuang gel.All the groups were treated for 4 weeks.The changes of the ear skin lesions of the rats in each group were observed during the modeling and intervention;the blood was taken from the rat heart in 24 hours after the last medication to detect the levels of serum IL-17 and IL-23 by ELISA;the rats in each group were sacrificed and the affected ear tissues were taken to be observed under the HE staining microscope.Results After the modeling,the ears of the rats in each group were swollen,the skin temperature increased,the epidermis was thickened and the texture was rough,with serious symptoms of desquamation and crust,and the hair follicle pores were enlarged.After the intervention,the rats in the low-dose Sanhuang gel group still had more uncured acne;the follicular keratotic plugging and nodules of the rats in the medium-dose Sanhuang gel group and the tretinoin cream group were significantly decreased,but still remained some desquamation;the ear skin of the rats in the high-dose Sanhuang gel group showed no swelling,desquamation and crusting,and basically returned to normal.The levels of serum IL-17 and IL-23 of rats in the model group were significantly higher than those of the blank group(all P<0.05);the levels of serum IL-17 and IL-23 of rats in each intervention group were significantly lower than those of the model group(all P<0.05),and the high-dose Sanhuang gel group was significantly lower than the tretinoin cream group(all P<0.05),there was no statistically significant difference in the low-dose,middle-dose group and the tretinoin cream group(all P>0.05).Under the light microscope,the expansion of the hair follicle opening was reduced in the low-dose Sanhuang gel group,in which the inflammatory cells were fewer than the model group;the hair follicle opening was relived and inflammatory cells were decreased in the middle-dose Sanhuang gel group and the tretinoin cream group;the inflammatory cells in the high-dose Sanhuang Gel group were significantly reduced.Conclusion Sanhuang gel can effectively treat for acne,and the mechanism may be related to the reduction of IL-17 and IL-23 expression.
