摘要：穴位敏化是穴位状态的相应改变,痛敏是穴位敏化最常见的表现形式,痛敏穴与激痛点在临床上均与疼痛有关,二者有许多相似之处,两者皆为疼痛或疾病的反应点,多用于治疗疼痛类疾病。从理论来源上讲,阿是穴可将痛敏穴与激痛点联系起来,并在疼痛敏感度和解剖位置方面为两者的临床应用提供更好的参考。在辅助诊治技术上,可应用各种成像技术的辅助进行痛敏穴、激痛点的可视化、标准化定位,并预测其疗效。从临床应用来看,针刺穴位以酸胀重麻感为准,针刺激痛点以扎跳为准,常根据患者的病情综合应用痛敏穴,激痛点的分布也可为临床选穴提供参考,两者可治疗的疼痛类疾病既包含躯体痛也包括非感染或外伤性的内脏痛。现代研究方面,两者都以发生炎性反应为特征,进而产生周围微环境的改变。文章从痛敏穴与激痛点在理论、位置分布、针感、检测方法、临床应用及现代研究6个方面的异同探讨二者的关系,并进行分析总结,阐明了穴位痛敏化现象与激痛点理论的相关性。通过梳理二者之间的关系,认为痛敏穴的范围更加广泛,激痛点应包含在痛敏穴之中。痛敏穴与激痛点是传统经络腧穴理论的现代科学研究,仍纳入中医经络腧穴理论体系;同时将两者贯通于现代针灸学的研究中,以期为两者在疼痛类疾病临床诊治中的应用提供思路与建议,以更好地指导临床治疗。
Acupoint sensitization is the corresponding change of acupoint state,pain sensitivity is the most common manifestation of acupoint sensitization.Pain sensitivity acupoint and pain trigger point are both clinically related to pain,and they have many similarities.The two are both response points of pain or disease,and are mostly used to treat pain diseases.Theoretically speaking,Ashi acupoint(阿是穴)can connect the pain sensitive acupoint with the pain trigger point,and provide a better reference for the clinical application of the two in terms of pain sensitivity and anatomical location.In terms of auxiliary diagnosis and treatment technology,various imaging technologies can be used in the future to assist in the visualized and standardized localization of pain sensitive acupoints and pain trigger points,and predict their curative effects.From the perspective of clinical application,acupuncture at pain sensitive acupoint is mainly based on soreness,bloat,heavy and numb sensations;acupuncture at pain trigger points is subject to muscle beating.Pain sensitive acupoints are often used compatibly according to the patient’s condition.The distribution of pain trigger points can also provide a reference for the clinical selection of patients.The pain diseases that can be treated by the two include both physical pain and non-infectious or traumatic visceral pain.In modern research,both of them are characterized by inflammation reactions,which lead to changes in the surrounding microenvironment.In this article,the similarities and differences between pain sensitive acupoints and pain trigger points are discussed from six aspects of theory,location distribution,needle sensation,detection method,clinical application and modern research to analyze and summarize the relevant information,and the correlation between acupoint pain sensitization phenomenon and pain trigger point theory is clarified.By sorting out the relationship between the two,it is believed that the scope of pain sensitive acupoint is relatively extensive,and the pain trigger points should be included in pain sensitive acupoints.Pain sensitive acupoint and pain trigger point belong to modern scientific research of traditional meridian acupoint theory,and are still included in the theoretical system of TCM meridian acupoint.At the same time,the two are integrated into the research of modern acupuncture and moxibustion,in order to provide ideas and suggestions for their application in the clinical diagnosis and treatment of pain diseases,so as to better guide clinical treatment.
