摘要：目的观察决银颗粒联合阿维A胶囊治疗寻常型银屑病血热证的临床疗效。方法将60例寻常型银屑病血热证患者采用随机数字表法分为对照组和治疗组,各30例。对照组口服阿维A胶囊治疗,治疗组在对照组治疗方法的基础上口服决银颗粒治疗。对照组剔除2例,治疗组剔除1例,连续治疗8周后比较2组的临床疗效、不良反应发生率以及治疗前后银屑病皮损面积和严重程度指数(PASI)评分与血清血管内皮生长因子(VEGF)、白细胞介素(IL)-22的含量。结果对照组总有效率为82.14%(23/28),治疗组为96.55%(28/29),2组比较差异有统计学意义(P<0.01);治疗后2组PASI评分及血清VEGF、IL-22含量均明显降低,与同组治疗前比较差异有统计学意义(P<0.01),且治疗组各指标降低更明显,与对照组治疗后比较差异有统计学意义(P<0.01);对照组不良反应发生率为10.71%(3/28),治疗组为3.45%(1/29),2组比较差异有统计学意义(P<0.01)。结论决银颗粒联合阿维A胶囊治疗寻常型银屑病血热证临床疗效显著,可明显降低患者PASI评分及血清VEGF、IL-22含量,且不良反应较小,值得临床推广应用。
Objective To observe the clinical efficacy of Jueyin Keli(决银颗粒)combined with acitretin capsules in the treatment of psoriasis vulgaris with blood-heat syndrome.Methods Sixty cases of psoriasis vulgaris with blood-heat syndrome were divided into control group and treatment group according to random number table method,with 30 cases in each.The control group was given oral acitretin capsules,on this basis,the treatment group was given oral Jueyin Keli.Two cases were excluded from the control group,while 1 case was excluded from the treatment group.After the 2 groups were both given consecutive treatment for 8 weeks,the clinical efficacy,the incidence of adverse reactions,psoriasis area and severity index(PASI)score,the levels of serum vascular endothelial growth factor(VEGF)and interleukin(IL)-22 before and after treatment of the 2 groups were compared.Results The total effective rate of the control group was 82.14%(23/28),that of the treatment group was 96.55%(28/29),and the difference was statistically significant between the 2 groups(P<0.01).After treatment,PASI score and the serum levels of VEGF and IL-22 of the 2 groups all significantly decreased,and the difference was statistically significant compared with the same group before treatment(P<0.01),moreover,various indexes of the treatment group decreased more significantly,and the difference was statistically significant compared with the control group after treatment(P<0.01).The incidence of adverse reactions of the control group was 10.71%(3/28),that of the treatment group was 3.45%(1/29),and the difference was statistically significant between the 2 groups(P<0.01).Conclusion It has marked clinical efficacy,can markedly reduce PASI score and serum levels of VEGF and IL-22 of the patients with less adverse reactions to use Jueyin Keli combined with acitretin capsules in the treatment of psoriasis vulgaris with blood-heat syndrome,which deserves clinical promotion and application.
