摘要：银屑病是一种慢性炎症性皮肤病。此病以红斑、丘疹、鳞屑损害为主要表现,病证顽固,反复发作,难以根治。此病病因病机尚不明确,中医认为其属于“白疕病”范畴,常以养血、润燥、祛风为基本治疗原则。甘肃中医药大学附属医院王思农教授经过多年研究与实践,认为肺与皮肤在生理、病理上联系密切,因此皮肤疾病应先治肺。文章总结了“从肺论治”的思想渊源和现代医学的关系,体现出皮肤病与肺病有着密切联系。王思农教授认为银屑病病机以“热、毒、瘀”为主,银屑病的证型以血热、血燥、血瘀三型为主,并结合不同阶段的症状表现,分为进行期、静止期、恢复期三期。王教授采用中西医结合治疗的方案,分期治疗银屑病,在治疗上,选用主入肺经的银翘消风散,疏风养血、清热祛湿,取得了显著效果。文章结合案例介绍王思农教授“从肺论治”银屑病的经验,以供临床参考。
Psoriasis is a chronic inflammatory skin disease characterized by erythema,papules,and scaly lesions.The disease is stubborn,recurrent,and difficult to cure.The etiology and pathogenesis of this disease are still unclear.TCM believes that psoriasis belongs to the category of Baibi(白疕)disease,and the basic treatment principle is to nourish blood,moisten dryness and dispel wind.After years of research and practice,Professor WANG Sinong from the Affiliated Hospital of Gansu University of Traditional Chinese Medicine,believes that lung and skin are closely related in physiology and pathology,so lung diseases should be treated first in the whole process of treating skin diseases.This paper summarizes the ideological origin of treatment from the lungs and the relationship between modern medicine,reflecting the close relationship between skin diseases and lung diseases.Professor WANG Sinong believes that the mechanism of psoriasis is mainly heat,poison and stasis,and the symptoms of psoriasis are mainly blood heat,blood dryness and blood stasis,and combined with the symptoms of different stages,it is divided into three stages:progressive,quiescent and convalescent periods,and the combination of TCM and Western medicine is adopted to treat psoriasis in stages.In terms of treatment,Professor WANG selected Yinqiao Xiaofeng San(银翘消风散),which can disperse wind and nourish blood,clear heat and remove dampness,and achieved remarkable results.This article introduces Professor WANG Sinong’s experience in treating psoriasis from the lung with cases for a clinical reference.
