摘要：查阅相关文献,系统总结了柴胡加龙骨牡蛎汤在肿瘤相关性抑郁症、产后抑郁症、脑卒中后抑郁症、冠状动脉粥样硬化性心脏病合并抑郁症、帕金森病合并抑郁症及糖尿病合并抑郁症等多种类型抑郁症中的临床应用,认为其单独或联合西药使用可提高临床疗效、改善患者生活质量并降低不良反应。同时,从调节下丘脑-垂体-肾上腺轴功能、改善单胺类神经递质水平、促进脑源性神经营养因子分泌及降低炎症因子表达等方面揭示了其抗抑郁的作用机制。目前研究存在药物配伍的协同作用及量效关系研究不够深入、临床疗效评估缺乏标准化检测指标等问题,未来需进一步探索方剂配伍的协同治疗作用,系统解析其作用机制,加强药物间量效关系研究,并完善临床疗效评估的标准化方法。
This study systematically reviews the clinical applications and mechanistic foundations of Chaihu Jia LongguMuliTang(柴胡加龙骨牡蛎汤,CLMT)inmanaging depression comorbidities includingcancer-relateddepression,postpartum depression,post-stroke depression,coronary heart disease with comorbid depression,Parkinson's disease-related depression,and diabetes-associated depression.Evidence confirms that CLMT monotherapy or its integration with Western pharmaceuticals significantly enhances therapeutic efficacy,improves quality-of-life metrics,and reduces adverse drug reactions.CLMT exerts antidepressant effects through multidimensional regulation of hypothalamic-pituitary-adrenal axis hyperactivity,augmentation of monoamine neurotransmitter levels,stimulation of brain-derived neurotrophic factor secretion,and suppression of inflammatory factor expression.Current limitations include insufficient research on synergistic mechanisms underlying herbal compatibility and their dose-response relationships,and lack of standardized efficacy evaluation protocols.Future research should prioritize the synergistic therapeutic effects of formula compatibility,systematically analyze its pharmacological mechanisms,enhance investigations into the dose-effect relationships among medications,and refine standardized methods for evaluating clinical efficacy.
