摘要：文章总结国医大师周信有教授论治溃疡性结肠炎的学术思想及临证经验。周老认为溃疡性结肠炎病机属“本虚标实”,本虚以先天禀赋不足、脾胃虚弱,标实为湿热瘀毒壅滞肠络,最终虚瘀互结致肠络损伤;治疗以“复方多法、综合运用、整体调节”为原则,临床强调补虚不碍邪、祛邪不伤正,寒温相济、通涩兼施,有效改善黏液脓血便及肠黏膜病变。周老的学术思想为溃疡性结肠炎的中医辨治提供了理论与实践参考,凸显了中医药多靶点、多途径、整体调节的特色,体现了中医药治疗炎症性肠病的优势所在。
This paper summarizes the academic thoughts and clinical experience of the national master of traditional Chinese medicine(TCM)ZHOU Xinyou in treating ulcerative colitis(UC).Professor ZHOU holds that pathogenesis of UC follows the pattern of“root deficiency with tip excess”.The root deficiency is attributed to constitutional insufficiency and dual deficiency and weakness of spleen and stomach,while the tip excess manifests as damp-heat and blood stasis toxins obstructing intestinal collaterals,ultimately leading to intestinal damage due to the entanglement of deficiency and stasis.Treatment follows the principle of“compound formulas and multiple methods,comprehensive application,and holistic regulation.”Clinically,it emphasizes reinforcing deficiency without retaining pathogenic factors,eliminating pathogens without damaging the healthy qi,balancing cold and heat,and integrating purgation with astringency.This approach effectively alleviates mucous-bloody stools and intestinal mucosal lesions.This study provides theoretical and practical references for TCM differentiation and treatment of UC,highlighting TCM multi-target,multi-pathway,and holistic regulation characteristics and its advantages in treating inflammatory bowel diseases.
