摘要:通过回顾艾灸缓解癌性疼痛的相关文献研究,发现艾灸缓解癌性疼痛具备双向良性调节的特点。艾灸既能通络止痛、荣血止痛,在临床方面表现为通过行气散寒、活血化瘀来治疗“不通则痛”;又能温阳培元、益气养血以缓解“不荣则痛”。艾灸调节癌性疼痛免疫微环境的作用机制主要体现在2个方面:一方面是正向调节,即改善免疫系统、提高免疫功能;另一方面是负向调节,即抑制炎症反应、修复炎性损伤。也就是说,艾灸既能发挥免疫促进作用,又能产生免疫抑制作用。这一理论为艾灸治疗癌性疼痛的应用与研究提供了有效的依据和思路。
By reviewing the relevant literature on moxibustion for alleviating cancer pain,it has been observed that moxibustion demonstrates a characteristic of bidirectional benign regulation in managing cancer pain.It canboth unblock meridians and nourish blood to alleviate pain,clinically manifested as treating“pain caused by stagna tion”by promoting qi circulation to dispel coldness and activating blood flow to resolve stasis.Meanwhile,it can warm yang energy to cultivate vitality and replenish qi-blood to relieve“pain caused by malnourishment”.The mechanism of moxibustion in regulating the immune microenvironment of cancer pain is mainly reflected in two aspects:positive regulation by improving the immune system and enhancing immune functions;and negative regulation by inhibiting inflammatory responses and repairing inflammatory damage.In essence,moxibustion exerts both immunostimulatory and immunosuppressive effects.This theoretical framework provides a solid foundation as well as valuable insights for the application and research of moxibustion in treating cancer-related pain.
