摘要:目的分析甘肃地区阿尔茨海默病患者和健康人群载脂蛋白E基因型分布,探讨阿尔茨海默病与载脂蛋白E的相关性。方法选取2018年10月-2022年6月在甘肃省人民医院神经内科确诊为阿尔茨海默病的患者63例作为阿尔茨海默病组,另收集同期体检中心健康人群96例作为对照组,通过聚合酶链式反应—限制性片段长度多态性技术对载脂蛋白E基因型进行检测,分析比较两组基因型和等位基因分布情况,并测定血浆中的生化指标和脑脊液生物标志物水平。采用SPSS 25.0软件进行数据分析。结果(1)阿尔茨海默病组和对照组载脂蛋白E基因型频率差异有统计学意义(P=0.04),阿尔茨海默病组和对照组等位基因总体构成比较,差异有统计学意义(χ^(2)=11.65,P=0.003)。对ε2、ε3、ε4等位基因频率分别进行组间比较,阿尔茨海默病组ε4等位基因频率高于对照组,差异有统计学意义(P<0.05),两组ε2、ε3差异无统计学意义(P>0.05)。(2)两组总胆固醇、空腹血糖、尿酸水平比较,差异有统计学意义(P<0.05);甘油三酯、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇及同型半胱氨酸水平在两组间差异无统计学意义(P>0.05)。(3)两组脑脊液生物标志物β淀粉样蛋白42、β淀粉样蛋白42/β淀粉样蛋白40比值、磷酸化τ蛋白及人神经轻链蛋白水平比较,差异有统计学意义(P<0.05),两组β淀粉样蛋白40与总τ蛋白水平比较,差异无统计学意义(P>0.05)。(4)Logistic回归分析表明体重指数是阿尔茨海默病的保护性因素(OR=0.854,95%CI:[0.759,0.961],P=0.009),收缩压(OR=1.029,95%CI:[1.007,1.053],P=0.011)及载脂蛋白Eε4(OR=3.147,95%CI:[1.484,6.674],P=0.003)是阿尔茨海默病的危险因素。结论ε4是阿尔茨海默病的危险因素,甘肃地区阿尔茨海默病患者与载脂蛋白E基因相关。
Objective To analyze the distribution of apolipoprotein E genotypes in patients with Alzheimer's disease and healthy people in Gansu province,and to explore the correlation between Alzheimer's disease and apolipoprotein E.Methods A total of 63 patients diagnosed with Alzheimer's disease in the Department of Neurology of Gansu Provincial People's Hospital from October 2018 to June 2022 were selected as the disease group,and 96 healthy people in the physical examination center during the same period as the control group.The apolipoprotein E genotype was detected by polymerase chain reaction-restriction fragment length polymorphism technique.The distribution of genotypes and alleles was compared between the two groups,and biochemical indexes in plasma and biomarkers in cerebrospinal fluid were determined.SPSS25.0 analysis software was used for data analysis.Results(1)There was significant difference in Apolipoprotein E genotype frequency between Alzheimer's disease group and control group(P=0.04),and also a significant difference in allele composition between the two(χ^(2)=11.65,P=0.003).The frequencies ofε2,ε3 andε4 alleles were compared between them,and the frequencies ofε4 alleles in the Alzheimer's disease group were higher than those in the control group,with the differences being statistically significant(P<0.05),whileε2 andε3 in the two groups were not(P>0.05).(2)There were statistically significant differences in the levels of total cholesterol,fasting blood glucose and uric acid between the two groups(P<0.05),while no statistically significant differences in the levels of triglyceride,high-density lipoprotein cholesterol,low-density lipoprotein cholesterol and homocysteine(P>0.05).(3)There were statistically significant differences in the ratio of cerebrospinal fluid biomarkersβ-amyloid 42,β-amyloid 42/β-amyloid 40,phosphorylated tau protein and neurofilament light chain levels between the two(P<0.05),but no statistically significant differences in the levels ofβ-amyloid 40 and total tau protein(P>0.05).(4)Logistic regression analysis showed that body mass index was the protective factor for Alzheimer's disease,while systolic blood pressure and Apolipoprotein Eε4 were the risk factors.Conclusionε4 is a risk factor for Alzheimer's disease.Apolipoprotein E gene is associated with Alzheimer's disease in Gansu Province.
