摘要：目的观察桃红四物汤加味治疗寻常型银屑病的临床疗效,以及对患者免疫功能、炎性因子的影响。方法将80例寻常型银屑病(血瘀证)患者随机分为两组,治疗组和对照组各40例,治疗组口服桃红四物汤加味、外用卡泊三醇软膏,对照组口服疗癣卡西甫丸和血府逐瘀丸、外用卡泊三醇软膏,治疗28天。观察两组患者治疗后PASI评分、中医症候评分、DLQI评分、免疫功能(CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+))及炎性因子(IL-6、IL-17、IL-22、TNF-α、IFN-γ)变化情况。结果治疗后,两组患者PASI评分、中医症候评分、DLQI评分、CD8^(+)、IL-6、IL-17、IL-22、TNF-α、IFN-γ水平较治疗前显著降低,CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平较治疗前显著升高(P<0.05);治疗后,治疗组患者PASI评分、中医症候评分、DLQI评分、CD8^(+)、IL-6、IL-17、IL-22、TNF-α、IFN-γ水平低于对照组,CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平高于对照组(P<0.05)。结论桃红四物汤加味可明显改善患者皮损、症状及免疫功能,降低炎症反应,提高患者生活质量,疗效较好,适合临床推广使用。
Objective To evaluate the clinical efficacy of modified Taohong Siwu Decoction(桃红四物汤,THSWD)in the treatment of psoriasis vulgaris(PV)and its influence on immune function and inflammatory factors.Methods Eighty patients with PV of blood stasis syndrome were randomly assigned to the treatment group and the control group,with 40 cases in each.The treatment group received oral modified THSWD combined with external use of calcipotriol ointment,while the control group received oral Liaoxuan Kaxifu Pill(疗藓卡西莆丸)and Xuefu Zhuyu Pill(血府逐瘀丸),along with external use of calcipotriol ointment.The treatment lasted for 28 days.Changes in Psoriasis Area and Severity Index(PASI)score,traditional Chinese medicine(TCM)syndrome score,Dermatology Life Quality Index(DLQI)score,immune function markers(CD3^(+),CD4,CD8^(+),CD4^(+)/CD8^(+)),and inflammatory factors(IL-6,IL-17,IL-22,TNF-α,IFN-)were assessed in both groups after treatment.Results After treatment,PASI score,TCM syndrome score,DLQI score,and CD8^(+),IL-6,IL-17,IL-22,TNF-α,and IFN-levels in both groups were significantly decreased,while CD3^(+),CD4^(+),and CD4^(+)/CD8^(+)levels were significantly higher than those of before treatment(P<0.05).The treatment group exhibited lower PASI,TCM syndrome and DLQI scores,as well as lower levels of CD8^(+),IL-6,IL-17,IL-22,TNF-α,and IFN-,but higher CD3^(+),CD4^(+),and CD4^(+)/CD8^(+)levels,compared to the control group(P<0.05).Conclusion Modified THSWD effectively alleviates skin lesions and clinical symptoms,modulates immune function,reduces inflammatory responses,and enhances patients'quality of life.Its favorable therapeutic effects make it suitable for clinical application.
