摘要:腰椎融合术的关键在于将融合器置入椎间隙以促进椎体间融合及维持腰椎稳定。对于多种脊柱疾病,如退行性疾病、创伤、肿瘤和畸形,这是一种成熟的手术策略。在过去的15年中,腰椎多种前外侧入路已被广泛应用,这些方法可微创进行且不会破坏椎旁肌和后脊柱,同时能够广泛暴露椎间盘空间,使用较大尺寸的融合器。本研究重点介绍3种微创前外侧入路:前路腰椎椎间融合术(anterior lumbar interbody fusion,ALIF),侧入路腰椎椎间融合术(lateral lumbar interbody fusion,LLIF),斜外侧腰椎椎间融合术(oblique lateral lumbar interbody fusion,OLIF),旨在探讨这3种手术方法对患者的选择、手术疗效、并发症及潜在的风险。
The key to lumbar fusion is to place the cage into the intervertebral space to promote interbody fusion and maintain lumbar stability.This is a mature surgical strategy for a variety of spinal diseases such as degenerative diseases,trauma,tumors and deformities.In the past 15 years,a variety of anterior and lateral approaches to the lumbar spine have been widely used.These methods can be performed minimally without destroying the paraspinal muscles and posterior spine,while extensively exposing the disc space,using larger cages. This review highlights three minimally invasive anterolateral approaches:anterior lumbar interbody fusion(ALIF),lateral lumbar interbody fusion(LLIF),and oblique lateral lumbar interbody fusion(OLIF).It aims to better understand and grasp the choice of patients,surgical outcomes, complications and potential risks of these three surgical methods.
