摘要：功能性消化不良是脑肠互动异常引起的以胃和十二指肠区动力障碍为特征的一个或一组症状,其发生的分子机制涉及能量代谢失调和胃肠Cajal间质细胞(ICC)的凋亡和功能障碍。线粒体自噬稳态失衡可导致氧化还原信号、凋亡信号传导和钙离子摄取等途径障碍,引起ICC凋亡及肌电节律紊乱,或直接干扰线粒体能量代谢,抑制胃肠平滑肌运动。功能性消化不良中医病机为本虚标实,“木郁土虚”是该病的基本病机。通过查阅大量文献发现,“木郁土虚”病机与线粒体自噬稳态失衡相契合,本文通过综述功能性消化不良木郁土虚病机和线粒体自噬相关内容,提出线粒体自噬紊乱是“土虚”的生物学基础,“木郁”是线粒体自噬缺陷的宏观体现,调控线粒体自噬是防治功能性消化不良的重要途径,以期为阐明抑木扶土法治疗功能性消化不良提供理论参考。
Functional dyspepsia is one or a group of symptoms characterized by gastric and duodenal motility disorders caused by abnormal brain-gut interaction.The molecular mechanism of functional dyspepsia involves dysregulation of energy metabolism and apoptosis and dysfunction of gastrointestinal Cajal interstitial cells(ICC).The imbalance of mitophagy homeostasis can lead to the dysfunction of redox signal,apoptotic signal transduction,calcium uptake and other pathways,which can lead to the abnormal apoptosis of ICC and the disturbance of myoelectric rhythm,or directly interfere with mitochondrial energy metabolism and inhibit gastrointestinal smooth muscle movement.TCM holds that functional dyspepsia is disease of excess in nature and deficiency in superficiality,“Mu Yu Tu Xu”(liver stagnation and spleen deficiency)is the basic pathogenesis of the disease.Through extensive literature review,it was found that the pathogenesis of“Mu Yu Tu Xu”is consistent with the imbalance of mitochondrial autophagy homeostasis.This article reviewed the pathogenesis of functional dyspepsia caused by liver stagnation and spleen deficiency and the related content of mitochondrial autophagy,and proposed that mitochondrial autophagy disorder was the biological basis of“spleen deficiency”,and“liver stagnation”was a macroscopic manifestation of mitochondrial autophagy defects,that regulating mitochondrial autophagy was an important pathway for preventing and treating functional dyspepsia,in order to provide theoretical reference for elucidating the treatment of functional dyspepsia with the method of suppressing liver and supporting spleen.
