摘要:目的分析2019年天水市健康体检人群血液指标分布情况及其与地理因素的关系,为制定适用于天水地区健康人群血液指标参考值范围提供依据。方法选取2019年在天水市人民医院体检中心进行体检的3150份健康成人静脉血标本,采用希森美康公司生产的SYSMEX XN-1000全自动血细胞分析仪对其进行血常规检测,并对结果进行统计学分析,探讨健康成人红细胞计数(RBC)、血红蛋白(Hb)、血小板计数(PLT)参数在不同年龄、性别间是否有差异及其与地理因素的关系。结果RBC和Hb升高比例在不同性别、年龄之间差异均有统计学意义,均P<0.001;PLT降低比例在性别间(P=0.530)差异无统计学意义,在年龄组间(P<0.001)差异有统计学意义。男性RBC与海拔高度(r=0.417,P=0.001)、年平均气温(r=-0.397,P=0.002)、年降水量(r=-0.362,P=0.006)、年日照时数(r=0.270,P=0.042)相关,与年平均相对湿度无关;女性RBC与海拔高度(r=0.518,P<0.001)、年平均气温(r=-0.328,P=0.013)、年降水量(r=-0.284,P=0.032)、年平均相对湿度(r=-0.275,P=0.038)相关,与年日照时数无关。男女性Hb均与海拔高度(r=0.517和0.505,均P<0.001)、年平均气温(r=-0.467和-0.437,均P<0.01)、年降水量(r=-0.483和-0.418,均P<0.01)、年平均相对湿度(r=-0.314和-0.320,P=0.016和0.013)相关,与年日照时数无关;男女性PLT与海拔高度、年平均气温、年降水量无关,与年平均相对湿度(r=-0.327和-0.381,P=0.019和0.006)、年日照时数(r=0.425和0.445,P=0.002和0.001)有关。结论RBC、Hb和PLT参考值范围与年龄、性别及部分地理因素指标有关,与操作规程规定的标准参考值范围存在差异,有必要针对天水地区重新建立参考值范围。
Objective To analyze the distribution of blood indicators among healthy people in Tianshui in 2019 and its relationship with geographical factors,so as to provide basis for formulating the reference value range of blood indicators for healthy people in Tianshui area.Methods Taking 3150 venous blood samples of healthy adults who underwent physical examination in the physical examination center of Tianshui People’s Hospital in 2019 as the research object,the SYSMEX XN-1000 automatic blood cell analyzer produced by Sison Meikang company was used for routine blood test,and the results were statistically analyzed to explore the changes of red blood cell count(RBC),hemoglobin(Hb)and platelet count(PLT)parameters of healthy adults at different ages whether there were differences between genders and their relationship with geographical factors.Results There were significant differences in the proportion of RBC and Hb increase between different sexes and ages(all P<0.001).There was no significant difference in the percentage of PLT reduction between sexes(P=0.530),but there was significant difference between age groups(P<0.001).Male RBC was correlated with altitude(r=0.417,P=0.001),annual average temperature(r=-0.397,P=0.002),annual precipitation(r=-0.362,P=0.006)and annual sunshine hours(r=0.270,P=0.042),but not with annual average relative humidity.Female RBC was correlated with altitude(r=0.518,P<0.001),annual average temperature(r=-0.328,P=0.013),annual precipitation(r=-0.284,P=0.032)and annual average relative humidity(r=-0.275,P=0.038),but not with annual sunshine hours.Both male and female Hb were correlated with altitude(r=0.517and 0.505,all P<0.001),annual average temperature(r=-0.467and-0.437,all P<0.01),annual precipitation(r=-0.483and-0.418,all P<0.01),annual average relative humidity(r=-0.314and-0.320,P=0.016and 0.013),but not with annual sunshine hours.Male and female PLT was not related to altitude,annual average temperature and annual precipitation,but related to annual average relative humidity(r=-0.327and-0.381,P=0.019and 0.006)and annual sunshine hours(r=0.425and 0.445,P=0.002and 0.001).Conclusions The reference value range of RBC,Hb and PLT is related to age,gender and some geographical factors.It is different from the standard reference value range specified in the operating procedures.It is necessary to re-establish the reference value range for Tianshui area.
